Picking up immediately where the first film ended, Marybeth (Danielle Harris) is attacked by Victor Crowley (Kane Hodder), but manages to escape. Jack Cracker finds her, pulls her out of the river, and takes her back to his cabin. After finding out her last name, he forces her to leave, telling her that if she wants help, she needs to see Reverend Zombie (Tony Todd). Moments after she leaves, Jack views Shapiro's camcorder, which he found in the swamp. The footage shows Shapiro filming different topless women, including Misty and Jenna. After Jack finishes watching the video, he is killed by Victor Crowley, who gouges his intestines out and strangles him with them until he decapitates him.

The minions of serial killer Jesse "ChromeSkull" Cromeans locate their employer and bring him back from the brink of death, though the injuries he sustained have left him disfigured. As ChromeSkull recuperates, tended to by an assistant named Spann, his disgruntled second-in-command, Preston, tracks Princess and Tommy, the survivors of the previous film, to a motel. The film picks up immediately after the end of the last film, with Marybeth Dunston (Danielle Harris) blowing off the head of Victor Crowley (Kane Hodder) with a shotgun. She walks away and comes across the bodies of Vernon and John, where she hears Crowley in the woods. Marybeth starts up the chainsaw, but is grabbed by Victor. Marybeth kicks him, and he falls backwards onto the chainsaw, cutting him down the middle. She grabs a shotgun from one of the men and shoots Victor's head, and walks back into the city. She walks into the Jefferson Parish Police Department, immediately having guns drawn on her. Upon discovery of the twenty to thirty bodies at Honey Island Swamp, she is placed as the prime suspect in the murders by Sheriff Fowler (Zach Galligan), even after telling him numerous times about the events of the last two films. The Sheriff heads out to the swamp with the paramedics and fire department, leaving Deputy Winslow (Robert Diago DoQui) in charge of the station until he gets back.

The film opens, showing that Mya is cheating on her husband Lewis with a man named Ben. Ben attempts to convince Mya to leave the city with him, but she remains noncommittal. As Mya exits, Ben turns the television on and watches a bizarre, psychedelic sequence of images. Mya begins to listen to a compact disc given to her by Ben, but she is menaced by men who are acting strangely in a parking garage. When she reaches her apartment building, she finds more people behaving strangely. Unknown to Mya, the Signal, a static-like interference that is coming through the television, radio and telephone amplifies each person's negative emotional traits, causing them to act irrationally and, in most cases, violently.
