Synopsis
Juan Oliver wants to make a good impression at his new job as a prison officer and reports to work a day early, leaving his pregnant wife, Elena, at home. During his tour of the prison, an accident occurs that knocks him unconscious. He is rushed to the empty but visibly haunted walls of cell 211. As this diversion unfolds, convicts break free and hijack control of the penitentiary. Aware of the violence that is to come, the prison officers flee, leaving Juan stranded and unconscious in the heart of the riot. When Juan awakens, he immediately takes stock of the situation; in order to survive, he must pretend to be a prisoner.Actors

The story follows the escape of two men from an Alaska prison, the efforts of a railroad dispatch office to safely stop the out-of-control train they are on, and the hunt by their warden to recapture them. Oscar "Manny" Manheim is a ruthless bank robber and hero to the convicts of Stonehaven Maximum Security Prison. After two previous escape attempts the doors to Manny's cell have been welded shut for three years. A court order compels Manny's nemesis, the vindictive and sadistic Associate Warden Ranken, to release him back into the general prison population. Ranken employs a serial killer to give Manny the incentive, by stabbing him grievously through his left hand. Jonah fatally stabs the serial killer to death in retaliation, and in turn is severely beaten by the prison guard, leaving him in a high-security hospital wing.

The film opens with two men running across a beach at dawn, it also appears that their hands are tied behind their backs. After several shots of a helicopter frantically searching the landscape, it becomes apparent that the two men are escapees of some kind. It is later revealed that their names are MacConnachie and Ansell. The two continue running across barren land, trying to escape the sight of the helicopter. MacConnachie continuously berates Ansell as they run, showing that he is the leader, more or less.

Nami Matsushima (Meiko Kaji) is set up by her boyfriend, a crooked police detective named Sugimi (Isao Natsuyagi) to win favor with the Yakuza. She is raped by several drug dealers. She makes a failed attempt to stab Sugimi on the steps of the Tokyo Metropolitan Police Headquarters. She is sentenced to do hard time in a women's prison. Matsushima is given the number 701. The prison is run by sadistic and lecherous male guards. The prisoners are forced to walk up and down a stair-like contraption naked with male guards watching from below. While in prison she meets inmates like Yuki Kida (Yayoi Watanabe) who was committed for fraud and theft, Otsuka (Akemi Negishi), jailed for burglary and extortion, and Katagiri (Rie Yokoyama) who has been impounded for arson and illegally disposing of a body. On the outside, Sugimi and the Japanese mafia orchestrate a plan in which Matsushima will succumb to an "accidental" death in prison. They enlist the help of Katagiri, pulling on her ties to both Sugimi and the mafia, and quickly set their plan in motion.

On January 18, 1960, Frank Morris (Clint Eastwood) arrives at the maximum security prison Alcatraz, having been sent there after escaping from several other prisons.
