Tina Balser, an educated, frustrated housewife and mother, is in a loveless marriage with Jonathan, an insufferable, controlling, emotionally abusive, social-climbing lawyer in New York City. He treats her like a servant, undermines her with insults, and belittles her appearance, abilities and the raising of their two girls, who treat their mother with the same rudeness as their father. The film opens a year after the previous film with Charles Garrison (Peter Coyote) arriving at a mortuary with one of the canisters of Trioxin that he salvaged from the previous movie. He is greeted by a team of Russian government officials whose goal is to destroy the last of the canisters to avoid another incident like that of what happened in Necropolis. Nevertheless, one of them sprays three corpses with the gas, and revives three corpses. Charles, is killed during the incident along with the mortuary owner and one of the government officials.

Ex-Marines Jimmy McGinnis (Victor McLaglen) and Harry Curtis (Edmund Lowe), who haven't seen each other for fifteen years, meet at a racetrack. Set 10 years after Return of the Living Dead 3, the film starts out with Charles Garrison (Peter Coyote) traveling to Chernobyl to collect the last 6 canisters of Trioxin 5. Two Russian sellers take him to an empty power station which contains the Trioxin. One of the Russians get Trioxin on his hands, turning him into a zombie. Upon hearing screams, Garrison runs to see the zombie eating the other Russian partner. Garrison promptly shoots the zombie in the head.

At Norfolk Naval Base in the opening months of World War II, Lieutenant John W. Harkness (Cooper), a newly commissioned officer, bids goodbye to wife Ellie (Jane Greer) and reports aboard the PC-1168 unaware that his civilian background in engineering and his Rutgers education has elected him, by means of a hole punched in an IBM card, to head a secret project and command the ship.
