It follows Aidan Breslin (Dennis Quaid) a bitter and emotionally distracted detective who has grown apart from his two sons after the death of his devoted wife. While investigating a series of murders he discovers a terrifying link between himself and the suspects that seem to be based on the Biblical prophecies concerning the Four Horsemen of the Apocalypse: War, Famine, Conquest and Death. The film was shot in Toronto, Winnipeg and Chicago, and was released on March 6, 2009.

Ross (Jason Schwartzman) is a customer of Spider Mike (John Leguizamo), a methamphetamine dealer. Spider Mike and his girlfriend Cookie (Mena Suvari) are constantly arguing, and Ross strikes up a friendship with Nikki (Brittany Murphy), a fellow addict. Nikki takes Ross to meet her boyfriend, "The Cook" (Mickey Rourke), who supplies Spider Mike with drugs from a meth lab he has set up in a motel room. The Cook gives a small amount of meth to Ross in exchange for bringing Nikki home, and says that he will get in touch with Ross if he needs a driver.

Diane suffers from chronic nosebleeds. She checks herself in a mirror and transforms into a monster before falling unconscious. Earlier, she walks in the streets trying to borrow a cellphone in order to call her twin sister Karen. Having no luck, she enters a clothing store and asks to use a phone. There, she meets a girl of her age, Jack, who is clearly smitten by Diane. Diane's nose starts bleeding again and Jack gives her a hand. Jack then takes Diane to a night club. When they get there, Diane seems nauseous, having lost a lot of blood from her nose, and she goes to the restroom. This scene is parallel to the beginning of the film because of the mirror. She gains consciousness, meets up with Jack and the girls passionately kiss.

After Zia commits suicide, he finds himself in an afterlife limbo much the same as life, just slightly worse. Here the color is dim, there are no stars, and no one can smile. This strange realm is populated by people who have committed suicide, such as Eugene, a Russian musician who lives with his mother, father, and brother – all suicide victims. Eugene kills himself on stage by pouring a beer he is drinking onto his electric guitar. Together they waste most of their afterlives in bars, until Zia learns from a friend, Brian, that his ex-girlfriend, Desiree, has also committed suicide. He and Eugene take to the road to find her, in Eugene's rundown car. Early on, it is revealed that Eugene's car has two idiosyncrasies: a sort of black hole underneath the passenger seat where items that are dropped disappear forever and broken headlights that cannot be fixed by the most adept of mechanics. On their trip, they pick up hitchhiker Mikal, who insists there has been a mistake, and she is seeking the "people in charge" (a.k.a. the PIC) in order to be sent back.

Teenager Mary Cummings (Jena Malone), who has "been born again her whole life," is about to enter her senior year at American Eagle Christian High School near Baltimore. The film is structured as a series of interconnected flashbacks. Following the death of his father Hal, Oliver reflects on their relationship following the death of Oliver's mother, Georgia. Shortly after her death, Hal came out to his son and began exploring life as an openly gay man. He becomes active in the gay community, finds love and becomes more honest with himself and with his son. As a result, Oliver and Hal become closer during this time. Shortly after Hal's death, Oliver meets Anna, a French actress, at a party. Gerard Reve (Jeroen Krabbé), an alcoholic, bisexual novelist, leaves Amsterdam to deliver a lecture at the Vlissingen Literary Society. There, he becomes sexually involved with its attractive treasurer, Christine Halslag (Renée Soutendijk), who is alternately described as a witch, black widow, Delilah and the Devil. The Virgin Mary appears to him in visions to show that he is targeted as her fourth victim. Mary says, "Anyone given a warning must listen to it." Gerard listens and his life is spared. He passes on the warning to Herman, Christine's other lover, who ignores it, thinking that Gerard is trying to scare him off so that he can have Christine for himself. The movie ends with Herman's death, Christine's selection of a fifth victim and Gerard's future uncertain.
