The title is derived from the source material for Apocalypse Now, the Joseph Conrad novella Heart of Darkness. Using behind-the-scenes footage, and narrated by Eleanor Coppola, it chronicles how production problems including bad weather, actors' health and other issues delayed the film, increasing costs and nearly destroying the life and career of Francis Ford Coppola. In 1990, Eleanor Coppola turned her material over to two young filmmakers, George Hickenlooper and Fax Bahr (co-creator of MADtv), who then shot new interviews with the original cast and crew and intercut them with her existing material. After a year of editing, Hickenlooper, Bahr, and Coppola debuted their film at the 1991 Cannes Film Festival to universal critical acclaim.

The film version is Jeff Garlin's documentary on the work of John Waters. Waters talks for an hour and a half about different subjects that made him into who he is today while standing on a stage decorated with a pile of trash, some roses, and a confessional. Waters starts off by talking about his earliest negative influences. He then begins talking about directors of the macabre that inspired him on what to do with his films. He then talks about filmmaking experiences on each and every one of his films and tells stories about some of the Dreamlanders. He discusses sexual fetishes, court trials he has visited, how to make books cool again, and more. The last topic he speaks of is his hometown of Baltimore and all the things he has experienced there.

Le documentaire retrace la vie de Glenn Milstead, dont le personnage de Divine a été sacré « plus belle femme du monde ». Avec ses amis habilleur et maquilleur, Milstead crée le personnage excessif et scandaleux de Divine qu'il interprète dans les films Mondo Trasho, Multiple Maniacs et Pink Flamingos. À la fin des années 1970, il se lance dans la chanson et fait plusieurs tournées aux États-Unis et en Europe. Il revient au cinéma en 1981 avec Polyester, puis dans Lust in the Dust de Paul Bartel et Wanda's Café. Peu après le succès de Hairspray, il meurt d'une crise cardiaque dans son sommeil.
