The main plot involves Dr. Caligari's experiments with her patients at the C.I.A (Caligari Insane Asylum) where she transfers glandular brain fluids from one patient to another. Two of her main patients, Mr. Pratt, a cannibalistic serial killer, and Mrs. Van Houten, a nymphomanical housewife, are the primary subjects of her mindswapping. Mrs. Van Houten becomes the cannibal and Mr. Pratt the nymphomaniac; although, they seem to still retain some elements of themselves as well. Apparently Caligari's unconventional idea is to cure people by introducing equally opposite traits to balance out disturbed minds, but this is never explicitly stated in the film. Several other doctors, a married couple Mr & Mrs Lodger, become concerned with Caligari's experiments and approach Mrs. Lodgers father Dr. Avol who confronts Caligari only to fall victim to her mindswapping and receives an injection of Mrs. Van Houtens fluid turning him into a transvestite nymphomaniac. Sex is a very prominent theme throughout the movie, especially for Mrs. Van Houten who appears topless and performs masturbation at several points, but there are no hard core graphic scenes as this was released as an R rated feature. By the end of the film Mrs. Van Houten has injected Dr. Caligari with her own nymphomanical fluid and herself with Caligari's ancestor's (the original Dr. Caligari from The Cabinet of Dr. Caligari) thus the patient becomes the doctor, the doctor becomes the patient and the inmates are left running the asylum.

The film focuses on Second Lieutenant Bart Gregory (David Anders) who has been killed under mysterious circumstances in Iraq. After his friends and girlfriend Janet (Louise Griffiths) attend his funeral, Bart awakens in his grave. Enlisting the help of his best friend, Joey Leubner (Chris Wylde), Bart begins to understand and learn how to deal with his new undead state; mainly, the fact that Bart needs blood to hold back decomposition and that he returns to a state of in-animation during daylight hours. Joey does research online to find out what Bart is and seems to be stuck between a Zombie and Vampire, finally stating that Bart is a Revenant.

Ten years passing by, Buppha is reincarnated as a young girl named Pla who is abandoned by her mother, leaving her with her barber stepfather, who often beats her in his anger. Mukundan Unni (Jayaram) is a tantrik magician and an exorcist. He is not really keen about his father's tantrik magic skills and is wiling away his time with two dumb cronies in a remote village. There he meets a girl named Malu (Poonam Bajwa), who has no memories about her past but falls pretty easily for the hero after a couple of songs. But they get separated soon after. Some years later, he is called by a wealthy family to exorcise a spirit called Rukku(Muktha George) from their palatial house, Shenoy Mandir. There he comes across Malu again under the name Chandana. He vows to get her back after she fails to recognise him. Mukundan then remembers his father's teachings and starts learning from the ancient thalliyolas passed from generation to generation. He manages to bring Malu (Rukku) to the manthrakallam. Here he finds out about Rukku's past; a girl from a poor background who loved a person from the Shenoy Mandir. When the three brothers in the Shenoy Mandir got to know of the affair, they killed of Rukku's family and burnt Rukku alive. Her lover, horrifyed at the sight, jumped into the flames and committed suicide. Since then Rukku became a vengeful soul and killed two of the three brothers. Mukundan was just about trap Rukku into a wooden doll when the Shenoy family came in, interrupting the pooja and freeing Rukku, who onece more possesses Malu. One night Malu(Rukku) tries to kill the third brother. But she is stopped by Mukundan and is exorcised, freeing her from the world. Without asking for anything in return, Mukundan starts to leave. Just as he was about to leave, the third brother gives Malu to Mukundan.

On their way to perform in Guam for the troops, nightclub performers Duke Mitchell and Sammy Petrillo find themselves stranded on a seemingly treacherous island, known by the natives as "Kola Kola".
