Smith is an 18-year-old film student who identifies sexually as "undeclared". He has been having strange dreams. He is going to college with his best friend, Stella, whom he has known since junior high. Smith finds a note saying that he is the "chosen son". He has a roommate, Thor, whom he lusts after, regretting that Thor is straight. Stella goes to a party with Smith, but hooks up with another girl, Lorelei. He recognizes Lorelei as one of the people in his dream. Smith notices a guy, but is distracted when a red-haired girl from his dream vomits on his shoe. The guy vanishes, but Smith gets picked up by London, a British student. They have sex, but to Smith's regret she does not want to be with him except during the sex.

Miriam Blaylock (Catherine Deneuve) is a beautiful and dangerous immortal vampire, promising specially chosen humans eternal life as her vampire lovers. As the film begins, her vampire companion is John (David Bowie), a talented cellist whom she married in 18th century France. The film opens in a night club in New York, to a live performance from Bauhaus. There they connect with another young couple who are brought home and fed upon by slashing their throats with a bladed Ankh pendant. In 1944 in the Republic of Salò, the Fascist-occupied portion of Italy, four wealthy men of power, the Duke, the Bishop, the Magistrate, and the President, agree to marry each other's daughters as the first step in a debauched ritual. They recruit four teenage boys to act as guards and four young soldiers (called "studs", "cockmongers", or "fuckers"), who are chosen because of their big penises. They then kidnap nine young men and nine young women and take them to a palace near Salò. Accompanying them are four middle-aged prostitutes, also collaborators, who recount arousing stories for the men, who sadistically exploit their victims.

In 1936 Tokyo, Sada Abe (Eiko Matsuda) is a former prostitute who now works as a maid in a hotel. The hotel's owner, Kichizo Ishida (Tatsuya Fuji), molests her, and the two begin an intense affair that consists of sexual experiments and various self-indulgences. Ishida leaves his wife to pursue his affair with Sada. Sada becomes increasingly possessive and jealous of Ishida, and Ishida more eager to please her. Their mutual obsession escalates to the point where Ishida finds she is most excited by strangling him during lovemaking, and he is killed in this fashion. Sada then severs his penis, walks around with it inside her for several days, and writes, "Sada Kichi the two of us forever," in blood on his chest.

Two mentally ill women who are clearly suffering from not otherwise specified delusional disorders, a mother and a (grown) daughter, live together in a secluded mansion. They are in a relationship and both are recognised for their beauty. They spend their days playing perverse BDSM-related incestuous games in memory of their sadistic patriarch who, when he was still alive, raped his daughter when she was eleven, murdered several servants, and is now a mummified corpse with which the daughter is shown having sex. Gerard Reve (Jeroen Krabbé), an alcoholic, bisexual novelist, leaves Amsterdam to deliver a lecture at the Vlissingen Literary Society. There, he becomes sexually involved with its attractive treasurer, Christine Halslag (Renée Soutendijk), who is alternately described as a witch, black widow, Delilah and the Devil. The Virgin Mary appears to him in visions to show that he is targeted as her fourth victim. Mary says, "Anyone given a warning must listen to it." Gerard listens and his life is spared. He passes on the warning to Herman, Christine's other lover, who ignores it, thinking that Gerard is trying to scare him off so that he can have Christine for himself. The movie ends with Herman's death, Christine's selection of a fifth victim and Gerard's future uncertain.
