Madhouse (original title: There Was a Little Girl; also known as And When She Was Bad) is a 1981 Italian slasher film directed by Ovidio G. Assonitis. It stars Trish Everly, Dennis Robertson, Allison Biggers, Michael Macrae, Morgan Hart, Edith Ivey and Jerry Fujikawa. The film features a musical score by Riz Ortolani and cinematography by Assonitis regular Roberto D'Ettorre Piazzoli.
It was one of the many films on the "video nasty" list, a list of horror/exploitation films banned by the BBFC in the 1980s for violence and obscenity.
Synopsis
Julia (Trish Everly, in her only film role) is a young teacher for deaf children living in Savannah, Georgia. She has horrid memories of her childhood, which was scarred by her sadistic twin sister Mary (Allison Biggers). At the urging of her uncle, Father James (Dennis Robertson), Julia visits Mary, suffering from a severe skin disease, in a mental institution. The meeting does not go well and Mary vows to make Julia "suffer as she had suffered". As their mutual birthday approaches, several of Julia's friends and neighbors begin to die gruesome deaths, some of which are committed by a mysterious Rottweiler dog that has some sort of connection to Mary. But is Mary really the killer?Actors

The sequel begins where the original left off. Skye Rotter has a nightmare that following the memorial for the teens murdered at the Roller-Dome (Madison, Olivia, Chloe, Lily and Kevin), Brigg Jenner and Derek McNamera have a run-in with her father, Charlie Rotter. In the dream, Rotter stabs Derek repeatedly in the neck and Brigg, seeing Skye as he is about to escape through a back door, is stabbed in the back. After waking, Skye drives away in Madison's BMW and takes it to a dealership. She exchanges the car and makes a deal with a shady dealer so that he won't call the police. She drives off and tries to start a new life in a small town hundreds of miles from her hometown.

The sequel picks up on Christmas Eve some years after the first one, with Ricky Caldwell, the 18-year-old brother of the first film's killer, is now being held in a mental hospital, awaiting trial for a series of murders that he committed. The film takes place in a theater late at night as a troupe of actors are rehearsing a musical about a mass murderer known as the Night Owl. When the show's leading lady, Alicia (Cupisti), sprains her ankle, she and the wardrobe mistress, Betty (Schwerk), sneaks out of the rehearsal to go to a doctor, in which the closest medical facility is a mental hospital. The psychiatrist on duty is happy to help, but the wardrobe mistress notices a restrained patient on a bed lying in a room behind bars. The doctor tells them that it is Irving Wallace (Clain Parker), a former actor who went insane and murdered dozen of people. Unbeknownst to any of them, Wallace killed one of the attendants with a syringe and sneaked out of the asylum, hiding in the back of Betty's car.

A young boy named Karl Berger (a surname given in the sequel) murders his abusive mother with a meat cleaver, after she beats him for returning home late. Twenty years later, in the mid-1970s, the imprisoned Karl is being transported to an unspecified location by the police, but manages to kill his captors and escape into the wilderness, somehow acquiring a cleaver in the process. Over the course of several days, Karl commits a series of murders across the countryside, mutilating and occasionally cannibalizing his victims. After one double homicide, Karl faints and has a flashback to the day he murdered his mother, revealing he had been coerced into killing her by a demon (which a line of dialogue indicates may be his father) he had encountered in the cellar after being locked in it.

Kris Fowles (Katie Cassidy) goes to the Springwood Diner to meet with her boyfriend, Dean Russell (Kellan Lutz), who falls asleep at the table and meets a man covered in burn scars, wearing a red and green sweater, a fedora and a clawed glove on his hand. The burned man cuts Dean's throat in the dream, but in reality it appears that Dean is cutting his own throat as his friend, waitress Nancy Holbrook (Rooney Mara), looks on with Kris. While making out on the beach, a couple is attacked by a blade-wielding man dressed like a hippie.
