The story is based on a tragedy that happened in Alummoottil Tharavadu, a famous central Travancore family, in the 19th century.The film dealt with an unusual theme which was not common in Indian cinema at the time but it proceeded to become a high grosser at the box-office and it was critically acclaimed as well.
Directors such as Siddique-Lal, Priyadarshan, and Sibi Malayil served as second-unit directors. The cinematography was by Venu and it was edited by T. R. Shekar. The film has an ensemble cast featuring Mohanlal, Shobhana, Suresh Gopi, Nedumudi Venu, Innocent, Vinaya Prasad, K. P. A. C. Lalitha, Ganesh Kumar, Sudheesh, and Thilakan in the main roles. The original songs featured in the movie were composed by M. G. Radhakrishnan, and while the original score was composed by Johnson. The film won the National Film Award for Best Popular Film Providing Wholesome Entertainment and Shobhana was awarded the National Film Award for Best Actress for her portrayal of the central character Ganga.
Apart from being the highest grossing Malayalam film of the year 1993, Manichitrathazhu is considered as one of the best thrillers ever made in India as well as one of the best Malayalam films ever made. Manichitrathazhu was remade after nearly 10 years in various languages, including Kannada (Apthamitra), Tamil and Telugu (dubbed) (Chandramukhi), Bengali (Rajmohol), and Hindi (Bhool Bhulaiyaa), all being commercially successful. Geethaanjali, a spin-off directed by Priyadarshan and Mohanlal reprising the role of Dr. Sunny Joseph, was released on 14 November 2013.

The Harikrishnans (Mammootty and Mohanlal) are one of the most famous lawyer duos in India. To avoid confusion one is called as Hari (Mammooty) and the other as Krishnan (Mohanlal). They head the organization called Harikrishnan Associates, which consists of around 300 lawyers. They get engaged in a murder case of Guptan, who was allegedly killed by Gabriel. Gabriel is a friend of Hari's sister and Harikrishnans become the defense lawyers upon her request. Harikrishnans begin investigation and come across Meera (Juhi Chawla), a friend of Guptan. Both of them fall in love with her. After some trouble over the matter, they rediscover their friendship and get involved in the case again. They discover that Guptan actually died due to poisoning by his relatives who were doctors. At the end Meera decides to go for a random method of choosing her lover as she liked both Harikrishnans equally. Two different climaxes of the film show either of Harikrishnans winning the toss for being Meera's friend; the losing person presumably becoming Meera's lover.

Nanda (Revathi) is in love with a successful architect Narendaran (Mohan Lal), both of whom are living in Bangalore. They get engaged and are all set to marry when tragedy strikes. While they are talking on top of a construction building, Narendran accidentally slips and falls down, dying instantly. Depressed Nanda escorts Narendran's corpse to his homeland in Kerala. To her surprise she sees Narendran's twin brother Krishnan Unni (Mohan Lal), who looks exactly like Narendran. She thinks Narendran is not dead and everyone is trying to fool her. Narendran's parents want Nanda to marry Unni but he is already in love with Bhadra (Shobana). Bhadra tries to commit suicide and this shakes Nanda, Bhadra's dear friend, and Unni. Not wanting to be a burden on anyone and truly missing Narendran, Nanda commits suicide. Narendran's ghost, moments before Nanda's suicide, tells Unni that he is going to take Nanda with him to heaven since he really needs her, even in death. Unni is unsure if he just had an illusion, but to the viewers, it shown that Narendran's ghost was present and actually talking to Unni.

Sivaraman (Mohanlal), an unemployed man, comes to Mumbai to find a job. Unfortunately he loses all his certificates but happens to find Govindan Kutty (Mukesh), who also is a jobless man (from Sivaraman's native place) wandering in Mumbai. They try to get visa from a travel agency but finds no way to gather such massive amount. Alina fakes to be in love with Nambeeshan to find the place where the money is hidden.

The story revolves around Manikyan (Mohanlal), Sreekrishnan(Nedumudi Venu) and Karthumbi (Shobana) and the love triangle between them. Manikyan works for Sreekrishnan and Sreekrishnan sees him like a brother. Once when they both are returning from a Mela after shopping, Sreekrishnan sees Karthumbi and gets attracted. Appukuttan Nair or 'Appu' (Mohanlal) is a struggling unemployed youth who arrives in Mumbai to meet his sister and brother-in-law (Augustine). His father has been falsely implicated in a bank fraud and wants to sell his ancestral property to pay towards the litigation. For that he needs his sister's approval, but is abused and turned away by his brother-in-law. Disappointed, Appu seeks out his old pal Nooruddin or 'Noor' (Sreenivasan), who is working in a fruit juice vending shop in Mumbai. Noor himself is a hapless uneducated youth working for his uncle Beeran (Mamukkoya), who is a treacherous money lender, with a hope to marry his daughter Maimoona. They plan to open a paint shop but need 1.25 lakh Rupees to rent a shop as prerequisite to get a bank loan. With no other way in sight, they swindle Beeran out of 1.25 lakh Rupees. Appukuttan then tries to get the loan sanctioned but he is rebuffed by a new Bank manager Varghese (Maniyanpilla Raju) who had taken charge by then.
