A French priest named Moreau warns the monks of a monastery about an impending attack by the devil's forces to obtain a boy named Danny. The monastery falls under attack; Moreau tries to help the boy and his mother Nadya escape, but the distrusting mother shoots at Moreau and flees with her son. Moreau manages to distract the men chasing Danny and Nadya, but nearly dies in the process and loses them. He believes that only the Ghost Rider is capable of protecting the boy. He finds the Rider and seeks his help.

Chev Chelios (Jason Statham) is a Chinese-based British hitman, working for a crime syndicate headed by Uglyman (Carlos Sanz). Chelios is contracted by Carlito to kill mafia boss Don Kim (Keone Young) as members of the Triads have been encroaching on Carlito's business. Carlito betrays Chelios by making it appear he assassinated Don Kim on his own, without his authorization. Ricky Verona (Jose Pablo Cantillo), an ambitious small-time criminal uses the opportunity to conspire with Carlito against Chelios; Verona will kill Chelios so the Triads do not retaliate, and then take Chelios's place as Carlito's new hired gun. While Chelios sleeps in his apartment, Verona, his brother Alex (Jay Xcala), and several henchmen break in and inject Chelios with a Chinese synthetic drug which inhibits the flow of adrenaline, slowing the heart and eventually killing the victim. Chelios wakes to find a recording left by Verona showing what he has done.

In 2024, inventor and professional computer programmer, Ken Castle (Michael C. Hall), has revolutionized the gaming industry with his self-replicating nanites that replace brain cells and allow full control of all motor functions by a third party. Castle's first application of this technology is a game called Society, which allows gamers to control a real person in a pseudo community (much like The Sims or Second Life). This allows players to engage in all manner of debauchery, such as deliberately injuring their "characters" and engaging in rough sex with random people. As a result, those who work as "characters" in Society are paid very well in compensation.

Immediately after the events of the previous film, Chev Chelios (Jason Statham) lands in the middle of an intersection after falling out of a helicopter. He is scooped off the street via snow shovel by a group of Chinese medics and removed from the scene. Chev wakes up in a makeshift hospital and sees doctors removing his heart while Johnny Vang (Art Hsu) watches. The doctors place Chev's heart in a white cooler with a padlock, and place a clear plastic artificial heart in his chest. He wakes up sometime later and escapes. He notices a yellow battery pack is attached to him. After a gunfight and interrogation of a thug, he learns the location of Johnny Vang: the Cypress Social Club.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. Five days after the end of the Civil War, John Wilkes Booth and Michael O'Laughlen, both members of the Knights of the Golden Circle (KGC), enter a tavern and approach Thomas Gates to decode a message. Thomas recognizes the message as using the Playfair cipher and begins to translate it. While he does so, Booth leaves for Ford's Theatre to assassinate President Abraham Lincoln. Thomas solves the puzzle and realizes the men are still loyal to the Confederacy. A fight breaks out, and Thomas rips several pages from the diary and throws them in the fireplace. Thomas is shot, and the gunman retrieves only a page fragment.
