In 1959, Alfred Hitchcock opens his latest film, North by Northwest, to considerable success, but is troubled by a reporter's insinuation that it is time to retire. Seeking to reclaim the artistic daring of his youth, Hitchcock turns down film proposals like adapting Casino Royale in favor of a horror novel called Psycho by Robert Bloch, which is based on the crimes of murderer Ed Gein. Gein appears in sequences throughout the film in which he seems to prompt Hitchcock's imagination regarding the Psycho story, or act as some function of Hitchcock's subconscious mind (for instance, drawing Hitchcock's attention to sand on his bathroom floor, the quantity of which reveals how much time his wife Alma has been spending at the beachhouse with Whitfield Cook).

The film begins by listing the headlining acts of the Super Rock festival held in Japan in 1984: Scorpions, Whitesnake, and Bon Jovi, all of whom have gone on to sell millions of records, except one: Anvil. Despite their ambition, the Canadian band was unable to achieve the same level of success. Instead, singer and guitarist Steve "Lips" Kudlow drives trucks for Children's Choice Catering, delivering food to schools and institutions. Drummer Robb Reiner works in construction. But both would rather be playing on stage at the local sports bar to their small but dedicated group of fans, as shown during a show for Steve's 50th birthday party.

Maladies is divided into chapters, titled "Feelings", "Symmetry", and "I See You". The course of the narrative lacks a specific timeframe; although the environment, setting and costumes appears designed for the early 1960s, repeated allusions to the cult leader Jim Jones in the film are somewhat jolting, as the footage of Jones used is from 1978. The film follows the foibles of its four main characters as they seek to resolve "maladies" that make it difficult for them to relate with one another and cope with themselves. James (James Franco) is an ex-TV star cum writer who finds himself unable to complete a novel. Patricia (Fallon Goodson), James' sister, is unable to speak. Une attaque suicide tue 15 personnes dans un supermarché de Kansas City. Après avoir assassiné un avocat du gang des Matamoros à Mexico, Gillick et Matt kidnappent Isabela, la fille du chef de cartel Carlos Reyes, lors d'une opération sous fausse bannière. Toutefois, les choses ne se passent pas comme prévu et Gillick se retrouve rapidement seul à devoir lutter pour sa survie et celle d'Isabela, abandonnés par le gouvernement américain et traqués par les cartels mexicains.
