Psycho III is a 1986 American psychological horror film. It is the second sequel to Alfred Hitchcock's Psycho and the third film in the Psycho series. The film stars Anthony Perkins (who also directed the film), Diana Scarwid, Jeff Fahey and Roberta Maxwell. The screenplay is written by Charles Edward Pogue. The original electronic music score is composed and performed by Carter Burwell in one of his earliest projects. The film was a financial failure, becoming the lowest grossing film in the Psycho series. It was followed by the TV movie, Psycho IV: The Beginning.

The film takes place one month after the events of Psycho II, Norman Bates is still running the Bates Motel with the corpse of Mrs. Spool still sitting up in the house. A suicidal nun, whom Norman falls in love with, comes to the motel along with a drifter named Duane Duke and a reporter who is trying to solve the mysterious disappearance of Emma Spool.

During a lunchtime tryst in Phoenix, Arizona, a real estate secretary named Marion Crane discusses with her boyfriend, Sam Loomis, how they cannot afford to get married because of Sam's debts. After lunch, Marion returns to work, where a client drops off a $40,000 cash payment on a property. Her boss asks her to deposit the money in the bank, and she asks if she can take the rest of the afternoon off. Returning home, she begins to pack for an unplanned trip, deciding to steal the money and give it to Sam in Fairvale, California. She is seen by her boss on her way out of town, which makes her nervous. During the trip, she pulls over on the side of the road and falls asleep, only to be awakened by a state patrol officer. He is suspicious about her nervous behavior but allows her to drive on. Shaken by the encounter, Marion stops at an automobile dealership and trades in her Ford Mainline, with its Arizona license plates, for a Ford Custom 300 that has California tags. Following a car accident, Paul Wagner suffers a head injury he is struggling to recover fully and can not manage his business of trading of champagne. Christine Belling takes advantage of the situation to try to sell the company, which Paul refuses to accept. Murders are committed: it is likely that Paul is the culprit. He then approaches Christine for her support. But she uses the situation to make Paul agree to the sale. Paul is the prime suspect in the eyes of investigators (who never actually make an appearance), due to brain damage suffered in an earlier accident and assault. Jacqueline, Christine Belling's secretary, is at the heart of this case, observing everything. Jacqueline is related to Christopher, husband of Christine. The dénouement is slightly Hitchcok-like, but the film's humour is meaner, and the view of humanity it fosters is bleaker.

Bill Chrushank (Jeff Fahey) is a psychologist working with convicted killers at a prison in addition to teaching classes at a university. Ray, a convict who murdered his cellmate recently, tells Bill he can’t do anything to fix him. At home, Bill wonders to his wife (Kim Delaney) if this is true. He wants to be able to fix someone like Ray but in all his research and studies has never seen proof it’s possible. The next day, while driving to work, Bill gets in a horrific car accident where he loses his arm. At the hospital, Dr. Agatha Webb (Lindsay Duncan) convinces Bill’s wife to sign off on an experimental transplant surgery to give him a new arm from a donor they have waiting. Heavily drugged, Bill is wheeled into the operating room where he groggily notes several armed cops in hospital scrubs. The cops leave as soon as Dr. Webb removes the unknown donor’s head from his body at a nearby operating table. Bill awakens from the surgery and struggles to rehabilitate his new arm, which is covered in gruesome scars. One day, while struggling to lift a tiny weight, his arm makes a violent unexpected move upward. After this happens Bill makes a swift recovery and is released from the hospital. After some initial awkwardness between his wife with the new arm, he pleasures her with it and they make love. Just when it seems things are back to normal, Bill starts seeing flashes of horrible acts of murder (as if he is committing them). Next, he loses control of his new arm and cuts his face while shaving. Then at the prison, Ray freaks out seeing that Bill has the same tattoo on his wrist, which is only given to inmates on death row. Bill has a police friend scan his new fingerprints and is shocked to discover the arm came from convicted serial killer Charley Fletcher (John Walsh), who had murdered 20 people.
