After miraculously recovering from a bullet wound to the head, Gulf War veteran Jack Starks (Adrien Brody) returns to Vermont in 1992, suffering from periods of amnesia. While walking, he sees a young girl, Jackie (Laura Marano), and her alcoholic mother (Kelly Lynch) in despair beside their broken-down truck. Starks and Jackie quickly form a certain affinity; she asks him to give her his dogtags and he does so. He gets the truck started for them and continues on his way. Shortly after, a man driving along the same highway gives Jack a ride and they get pulled over by a policeman. The scene changes: Starks is found lying on the deserted roadside near the dead policeman, with a slug from the policeman's gun in his body. The murder weapon is on the ground nearby. Although he testifies there was someone else at the scene, he is not believed because of his amnesia. Starks is found not guilty by reason of insanity and is incarcerated in a mental institution.

Tom Ludlow (Keanu Reeves) is a disillusioned LAPD Vice detective working for a unit known as Vice Special and haunted by the death of his wife. Working undercover, he meets with Korean gangsters (whom he believes have kidnapped two Korean schoolgirls) in a parking lot, who are looking to buy a machine gun from him. After a vicious beatdown, the Koreans then proceed to steal Tom's car. Tom however planned on this and has the cops locate the vehicle via GPS. Upon arrival at their hideout, Tom storms in and kills the four inside with his .45 Smith & Wesson 4506 pistol, and then locates the missing children after covering up what really happened. While the other officers in his unit congratulate him, he is confronted by his former partner, Detective Terrence Washington (Terry Crews). The film begins with the sentencing of Jean-Baptiste Grenouille (Ben Whishaw), a notorious murderer. Between the reading of the sentence and the execution, the story of his life is told in flashback, beginning with his abandonment at birth in a French fish market. Raised in an orphanage, Grenouille grows into a strangely detached boy with a superhuman sense of smell. After growing to maturity as a tanner's apprentice, he makes his first delivery to Paris, where he revels in the new odors. He focuses on a redheaded girl (Karoline Herfurth) selling yellow plums, following her and repeatedly attempting to sniff her, but startles her with his behavior. To prevent her from crying out, he covers the girl's mouth and unintentionally suffocates her. After realizing that she is dead, he strips her body naked and smells her all over, becoming distraught when her scent fades. Afterwards, Grenouille is haunted by the desire to recreate the girl's aroma.

The film examines the mind of anthropologist Ethan Powell (Hopkins) who has been missing for a few years, living in the jungle with gorillas. He had been convicted of killing and injuring several supposed Wilderness Park Rangers in Africa, and is sent to prison. A bright young psychiatrist (Gooding) tries to find out why he killed them, but becomes entangled in a quest to learn the true history and nature of humankind. Eventually it is revealed that during the course of his stay with the gorillas, the animals accepted Powell as part of their group and that he was attempting to protect his gorilla family when the rangers arrived and started shooting them. He gets a hearing to reveal the truth, but an attack by a vicious guard causes Powell to stop talking again. At the end of the film, Powell escapes from prison, using a pen to dig out the lock on a window and heads back to Africa.
