She makes a failed attempt to stab Sugimi on the steps of the Tokyo Metropolitan Police Headquarters. She is sentenced to do hard time in a women's prison. Matsushima is given the number 701. The prison is run by sadistic and lecherous male guards. The prisoners are forced to walk up and down a stair-like contraption naked with male guards watching from below. While in prison she meets inmates like Yuki Kida (Yayoi Watanabe) who was committed for fraud and theft, Otsuka (Akemi Negishi), jailed for burglary and extortion, and Katagiri (Rie Yokoyama) who has been impounded for arson and illegally disposing of a body. On the outside, Sugimi and the Japanese mafia orchestrate a plan in which Matsushima will succumb to an "accidental" death in prison. They enlist the help of Katagiri, pulling on her ties to both Sugimi and the mafia, and quickly set their plan in motion.

The Virgin Mary appears to him in visions to show that he is targeted as her fourth victim. Mary says, "Anyone given a warning must listen to it." Gerard listens and his life is spared. He passes on the warning to Herman, Christine's other lover, who ignores it, thinking that Gerard is trying to scare him off so that he can have Christine for himself. The movie ends with Herman's death, Christine's selection of a fifth victim and Gerard's future uncertain.

A retired rock star, Johnny Boz, is stabbed to death with an ice pick during sex by a mysterious blonde woman at his apartment. Homicide detective Nick Curran investigates, and the only suspect is Catherine Tramell, Boz's bisexual girlfriend and a crime novelist who has written a novel that mirrors the crime. It is concluded that Catherine herself did it or someone who is trying to frame her out of spite. Tramell is uncooperative and taunting in the investigation, smoking in the interrogation room and exposing her bare genitalia in front of the officers. She presents alibis and passes a lie detector test. Nick discovers that Catherine has a habit of befriending murderers, including her girlfriend Roxy, who is later shown to have murdered several young boys on impulse, and Hazel Dobkins, who murdered her family.

The film opens in New York City, where Bridget works as a telemarketing manager and her husband Clay is training to be a doctor. He is heavily in debt to a loan shark so he arranges to sell stolen pharmaceutical cocaine to two drug dealers. The transaction becomes tense when the buyers pull a gun, but rather to Clay's surprise, they eventually pay him $700,000. Clay is left shaken, and on his return home he slaps Bridget after she insults him. She then steals the cash from him and flees their apartment while he is in the shower.
