Paris in the 1880s. Georges Duroy (Robert Pattinson) is a penniless soldier who has spent the last six months in Paris, barely making a living as an office clerk. At a club he meets an old friend, Charles Forestier (Philip Glenister), with whom he spent three years in the war in Algeria. The friend is well off and invites him to his home where he meets Mrs. Madeleine Forestier (Uma Thurman) and her friends Clotilde de Marelle (Christina Ricci) and Virginie Rousset (Kristin Scott Thomas). Mrs. Rousset's husband is an editor of the conservative newspaper La Vie Française and she helps him to get a job there, initially by publishing his diaries from the war in the paper. Step by step he finds ways improve his social and financial status. He uses his wit and powers of seduction to charm wealthy women, and as he goes from nothing to money all hell breaks loose and things happen and end up looking bad for him.

The Cantones are a bourgeois family who own a pasta factory in Lecce in conservative Southern Italy. Tommaso discusses his plans with his elder brother, Antonio (Alessandro Preziosi), and also tells him about his desire to break away from the family business to become a writer in Rome. During a dinner with the Brunetti family, with whom the Cantones are in business, just as Tommaso is about to share his news, Antonio interrupts and reveals to everyone that he himself is homosexual. Mr Cantone (Ennio Fantastichini), the brothers' father, takes the news badly and is admitted to hospital.

The film opens on a graffiti-covered wall with Encolpius lamenting the loss of his lover Gitón to Ascyltus. Vowing to win him back, he learns at the Thermae that Ascyltus sold Gitón to the actor Vernacchio. At the theatre, he discovers Vernacchio and Gitón performing in a lewd play based on the "emperor's miracle": a slave's hand is axed off and replaced with a gold one. Encolpius storms the stage and reclaims Gitón. On their return to Encolpius's home in the Insula Felicles, a Roman tenement building, they walk through the vast Roman brothel known as the Lupanare, observing numerous sensual scenes. They fall asleep after making love at Encolpius's place. Ascyltus sneaks into the room, waking Encolpius with a whiplash. Since both share the tenement room, Encolpius proposes they divide up their property and separate. Ascyltus mockingly suggests they split Gitón in half. Encolpius is driven to suicidal despair, however, when Gitón decides to leave with Ascyltus. At that moment, an earthquake destroys the tenement.

The action of the film takes place in 1978, in a fictional town called Acqua Traverse in Southern Italy, during the hottest summer of the century and the infamous Years of Lead. A nine-year-old boy named Michele Amitrano (played by Giuseppe Cristiano) and a group of his friends set out on a race across scorched wheat fields to a deserted farmhouse. Michele's sister tags along but falls over, breaking her glasses, and she calls out to Michele, who runs back to her. Michele quickly calms her worries about the glasses, and they continue running. They are the last of the group to arrive at the farmhouse, which means that she and Michele must pay a forfeit. However, the leader of the group, Skull, chooses the only girl in the group apart from Michele's sister to pay up instead. He instructs her to expose herself to the boys, and she looks to the others for help, but they refuse to meet her gaze. She reluctantly and hesitantly begins to take off her clothes, when Michele pipes up that he was the one to arrive last and he should be the one to pay.

Rome, Year of our Lord 1809. The Pope Pius VII with his cardinals and ministers manages both temporal and spiritual power of the Papal States in Italy. The Marquis Onofrio del Grillo is one of his favorites, but even the worst of all the nobility. As a privileged and protected nobleman, Onofrio feels free to play his pranks on the poor people without any fear of the consequences. On one occasion, when he is arrested at a dinner with common criminals, he turns to the populace in a vulgar speech, claiming that his nobility allows him to do what he wants, and that they, being poor, are not worth anything. Memorable is the dispute between the poor Jew Aaron Piperno and the Marquis for the payment of a salary. Aaron is amazed when Onofrio refuses to pay with the argument that his creditor being a Jew is a murderer of Jesus. Aaron brings his case to court, but Onofrio wins the lawsuit by corrupting the judges and the cardinals. Piperno is condemned and mocked by the people, and Onofrio announces to the Pope that justice has just died in his States.

Irena (Kseniya Rappoport), a Ukrainian prostitute on the run, is determined to find a job in an elegant apartment building in northern Italy, and starts by cleaning the stairs. She does it in order to inch her way into working for a family residing in that building. She befriends Gina (Piera Degli Esposti), the nanny of the family's child, Thea (Clara Dossena), who also lives with them in their apartment. When the nanny is crippled in a fall—tripped by Irena—but presumed to be accidental, Irena is hired to take her place. Through flashbacks, viewers learn that Irena has been physically and emotionally abused, and forced to bear nine children, all taken away at birth to be sold to adoptive families. After stabbing her pimp and leaving him for dead, she sets out to find her youngest child, whom she believes is Thea; hence the plot to work for them. Adoption documents in the apartment convince her that Thea is indeed her daughter. The mother grows suspicious of Irena and fires her, despite the loving relationship that has grown up between Irena and the child. Irena's pimp stalks her and sends out thugs to beat her up as she walks down the street. He rigs her employer's car, leading to a crash in which Thea's mother is killed. The pimp forces Irena to drive him to a location that may or may not contain the money Irena stole from him when she left him for dead. During a struggle, he falls and is killed as his head hits a rock. Thea's father moves to a new apartment and prepares a room for Irena, but as the police suspect foul play around the death of Thea's mother, Irena is taken into custody. She reveals both that the true culprit is the pimp and that she killed him. She is tried, and sent to jail. Thea stops eating until the judge allows Irena to visit her in the hospital and feed her. DNA testing reveals that Thea is not her daughter after all. After she is out of jail, she finds Thea, a young lady now waiting for her.

The two lost touch during the war years and, upon discovering him again in a hospital during the war, she also discovered Rachele Guidi, who had married Mussolini in 1915, and a daughter born in 1910 when Guidi and Mussolini were living together.
