High school student Kale Brecht and his father Daniel get into a serious car accident while driving home from a fishing trip, and Daniel dies. One year later, Kale remains inconsolable after his father's death. Near the end of the school year, he is reprimanded by his Spanish teacher, Señor Gutierrez, and when Gutierrez brings up Kale's father, Kale attacks him. For this assault, he is sentenced by the sympathetic judge to a three-month house arrest period with an ankle monitor and a proximity sensor, which prohibit him from roaming beyond the boundaries of his house and yard. He then learns that one of the police officers monitoring him is his teacher's arrogant cousin. Initially, he satiates his boredom by playing video games, but shortly after, his mother Julie cancels his subscriptions to the iTunes Music Store and Xbox Live, and cuts the power cord of his television in order to get him to learn a hard lesson.

Telly Paretta (Julianne Moore) believes that her son, Sam (Christopher Kovaleski), died fourteen months ago in a plane crash, but her husband Jim (Anthony Edwards) tells her that she's delusional and that they have never had a son, and Eliot (Jessica Hecht) doesn't appear to believe in Sam's existence despite her closeness to him. Dr. Munce (Gary Sinise) tells her that Sam was merely a figment of her imagination and is just imagining a life that might have been. He recommends that she be sent to a hospital, but she runs away and meets with a man named Ash (Dominic West) who she thinks is the father of a girl (Kathryn Faughnan) who was friends with her son and died in the same crash. At first he dismisses her, claiming he never had a daughter, and calls the police. After she is taken into custody, however, he remembers his daughter and rescues Telly. Together they escape and go into hiding, pursued by National Security agents.

The film begins in September 2008 (opening against the backdrop of news of the Islamabad Marriott Hotel bombing) with an elderly Lady Thatcher buying milk unrecognized by other customers and walking back from the shop alone. Over the course of three days, we see her struggle with dementia and with the lack of power that comes with old age, while looking back on defining moments of her personal and professional life, on which she reminisces with her (now-dead) husband, Denis Thatcher, whose death she is unable to fully accept. She is shown as having difficulty distinguishing between the past and present. A theme throughout the film is the personal price that Thatcher has paid for power. Denis is portrayed as somewhat ambivalent about his wife's rise to power, her son Mark lives in South Africa and is shown as having little contact with his mother, and Thatcher's relationship with her daughter Carol is at times strained.

Convicted murderer Seth Baxter wakes up chained to a table beneath a pendulum blade. A videotape informs him that he can release himself by crushing his hands between two presses. He does so, but the blade still swings down and cuts him in half as someone watches through a hole in the wall. In the meatpacking plant, FBI Agent Peter Strahm guns down Jeff Denlon and is locked in the sickroom. He discovers a hidden exit with a microcassette recorder, which urges him to stay in the sickroom. He ignores it and is attacked in the tunnel by a pig-masked figure, waking up with his head sealed in a box which quickly fills with water; he survives by performing a tracheotomy using his pen. Outside, Detective Mark Hoffman delivers Corbett Denlon to the police and claims they are the only survivors, and is shocked when Strahm is also brought out alive.

Adam, a young photographer, awakens in a filthy bathtub. He gets out and finds himself chained to by one foot to a pipe in a large dilapidated bathroom. Lawrence Gordon, an oncologist, is chained by one foot to a pipe across the room and between them lies a corpse, holding a revolver and a microcassette recorder. Adam manages to retrieve the recorder and they learn from tapes in their pockets what they must do: Adam must escape the bathroom, and Lawrence must kill Adam before six o'clock, or his wife and daughter will die and he will be left to die as well. Using a clue from the tape, Adam finds a bag in the toilet tank containing two hacksaws, though neither is able to cut through their chains. Adam's saw breaks and he throws it at a mirror in frustration. Lawrence realizes the saws are meant to be used on their feet and identifies their captor as the Jigsaw Killer, whom Lawrence knows of because he was once a suspect.
