An alcoholic air marshal named Bill Marks boards a Boeing 767 non-stop flight from New York City to London. He sits next to passenger Jen Summers with whom he engages in friendly conversations. After take-off, he receives a text message on his secure phone stating that someone will die every 20 minutes unless $150 million is transferred to a specific bank account. Marks breaks protocol and consults with Jack Hammond, the other air marshal, who concludes that the threat is not valid. Marks however consults the help of Summers and flight attendant Nancy to monitor the security cameras while texting the mysterious person in order to locate him. However, Hammond is texted by the unknown person who says that he knows what is in Hammond's briefcase. As Marks confronts Hammond in the rear lavatory, the latter is revealed to be smuggling cocaine in a brief-case. Hammond attacks Marks, who responds in self-defense and breaks Hammond's neck. This incident happens exactly at the 20-minute mark.

In the Middle Ages, Vlad Tepes (Luke Evans) is the Prince of Wallachia and Transylvania. As a child, he was a princely hostage of the Ottoman Empire and was trained to be a soldier in the Sultan's elite Janissary corps, where he became their most feared warrior. He was called Vlad the Impaler after slaughtering thousands by impaling them on spears. Eventually sickened by his acts, he put aside his past and returned to rule his domains in peace. One day in the forest, Vlad and his soldiers discover a helmet in a stream. Fearing that an Ottoman scouting party is preparing the way for invasion, they follow the stream to a high mountain cave called Broken Tooth Mountain. Upon entering the cave, they discover the ground of the cave is carpeted in crushed bone; and they are attacked in the dark by an unknown creature (Charles Dance) with shining red eyes, as well as inhuman speed and strength. While his men are killed, Vlad cuts the creature of the cave with his sword before being thrown to the ground. The blood on the sword he used to cut the creature dissolves when exposed to sunlight, and the creature does not pursue Vlad into the sunlight at the mouth of the cave. Returning to his castle, Vlad learns from a local monk that the creature is a vampire, who was once a man who summoned a demon from the depths of hell and made a pact with it for dark powers before being tricked by the demon and cursed to remain in the cave forever until he is released by someone who drinks his blood, then drinks the blood of a human. The person is welcome to share his power in exchange for the freedom to escape the cave.

The Expendables—led by Barney Ross and formed by Lee Christmas, Gunnar Jensen, and Toll Road—extract former member Doctor Death, a knives specialist and team medic, from a military prison during his transfer on a train. They recruit Doc to assist them in intercepting a shipment of bombs meant to be delivered to a warlord in Somalia. Arriving there, they reunite with Hale Caesar, who directs them to the drop point, where Ross is surprised to find out that the arms trader providing the bombs is Conrad Stonebanks, a former co-founder of the Expendables who went rogue and was presumed dead. In the ensuing firefight, The Expendables manage to kill all but Stonebanks, who shoots Caesar twice. As the team attempts to aid him, they are forced to retreat due to Stonebanks' advanced weaponry, and Caesar is severely injured in the process.

In 1988, Colonel Sam Trautman (Richard Crenna) visits his old friend and ally John Rambo (Sylvester Stallone) in Thailand. British agent 009 is found dead at the British embassy in East Berlin, dressed as a circus clown and carrying a fake Fabergé egg. MI6 immediately suspects Soviet involvement and, after seeing the real egg appear at an auction in London, sends James Bond—agent 007—to investigate and find out who the seller is. At the auction Bond is able to swap the real egg with the fake and engages in a bidding war with exiled Afghan prince Kamal Khan, forcing Khan to pay £500,000 for the fake egg. Bond follows Khan back to his palace in Rajasthan, India, where Bond defeats Khan in a game of backgammon. Bond escapes with his Indian colleague Vijay, evading Khan's bodyguard Gobinda's attempts to kill them both. Bond is seduced by one of Khan's associates, Magda, and notices that she has a blue-ringed octopus tattoo. Magda steals the real Fabergé egg fitted with a listening device by Q, while Gobinda captures Bond and takes him to Khan's palace. After Bond escapes from his cell he listens in on the bug in the Fabergé egg and discovers that Khan is working with Orlov, a Soviet general, who is seeking to expand Soviet control into Central Europe.
