Now the legal guardian of younger brother Jimmy (Andre Kinney), the Silver Star recipient struggles to make ends meet. In a bar after an armored car prank heist played on Ty, his co-workers recount several historical attacks on armored cars. It is explained that Ashcroft (Fred Ward), their immediate supervisor, was once involved in an ambush, becoming the only survivor as he single-handedly killed five assailants. This story was topped by "a better" one of an armored car jacking in '88, wherein the guards surrendered the funds they were escorting in order to prevent the killing of a fellow guard; it concluded in the safety of all the guards and escape of the hijackers. The thieves were never caught, and the crew speculates that the escort team staged the attack.

A television reporter named Angela Vidal (Jennifer Carpenter) and her cameraman Scott Percival (Steve Harris) are assigned to follow firefighters Jake (Jay Hernandez) and Fletcher (Johnathon Schaech) on their night shift. The crew responds to an emergency call from an apartment building. The apartment manager Yuri (Rade Sherbedgia) says that he and other residents heard screams from the room of an old woman named Mrs. Espinoza (Jeannie Epper), who has locked herself in her apartment. Jake, Fletcher, Yuri, police officers Danny (Columbus Short) and James (Andrew Fiscella), and the camera crew go to the apartment, where they find Mrs. Espinoza in serious condition. She is bleeding severely and foaming at the mouth. Moments later, they are attacked by Espinoza who bites James in the neck. They take him downstairs for medical assistance, while Fletcher stays with a now sedated Mrs. Espinoza upstairs. However, when they get downstairs, they find the apartment doors have been locked from the outside, leaving everyone, including several residents, trapped inside. Upstairs, Fletcher is attacked by Mrs. Espinoza and thrown from the railing to the ground, leaving him in serious condition. Lawrence (Greg Germann), a veterinarian, starts tending to the injured as best as possible.

Set three years before the first film, the action takes place in and around the Meadow View Inn, a small roadside motel in rural North Carolina. It opens with newlyweds driving and the bride wants to have sex in the car, which makes the groom stop. They are then stopped by a man, whose property they are on. The manager, Gordon (David Moscow), oversees an operation of using cameras within the motel to videotape the newlyweds having oral sex. Later, a serial killer (Scott G. Anderson), who checks into the motel as Smith brutally murders a woman he brought with him in one of the motel rooms. The horrified staff see the whole crime on videotape and then subdue the man. They, however, do not know what to do with him as they will endanger their operation if they call the police. Smith manages to convince Gordon and his staff to hire him to torture and murder the motel guests on videotape and then sell them as snuff films.
