The film takes place in 1986, 13 years after the events of the first film. It begins with Buzz and Rick, two rowdy high school seniors racing along an abandoned stretch of Texas highway, en route to a weekend of fun in Dallas. They are heavily intoxicated and use their car phone to call and harass on-air radio DJ Vanita "Stretch" Brock (Caroline Williams). Unable to convince them to hang up, Stretch is forced to keep the line open. Buzz and Rick encounter a large pickup truck which runs parallel to them on a remote bridge. Suddenly, Leatherface (Bill Johnson), wielding a chainsaw, emerges from the back of the truck and proceeds to attack Buzz and Rick. After a short struggle, Rick tries to shoot Leatherface with a .44 Magnum revolver, but misses his target. Leatherface then slices off part of the driving Buzz's head, and the car ends up crashing and killing Rick.

Daniel Ray (Robert A. Burns) has been arrested and much to the surprise and horror of the police officers, he begins to confess to the murders of over 200 women. He also talks about his background as a childhood abuse victim and runaway before meeting Moon (Dennis Hill) and Molly (Sidney Brammer), who accompanied him on several of his murders. This proves to be too much for Sheriff Will Gaines (Berkley Garrett) to believe, only for Daniel Ray to describe the events that led to his capture and arrest.

The film tells the story of missionary Nathaniel "Nate" Williamsen (O'Keefe), taken to an island mission with his fiancee Sophie (Seagrove). Their ship, the Rona, is captained by the roguish Bully Hayes (Jones), who also takes a liking to Sophie. When Sophie is kidnapped by slave trader Ben Pease (Max Phipps), "Nate" teams with Hayes in order to find her. The plot is essentially a set-up for a rousing series of Indiana Jones style action set pieces, including a sequence on a suspension bridge which greatly resembles the climax of Indiana Jones and the Temple of Doom, though Nate and Hayes was released a year earlier.
