Brothers Brian and Matt, Matt's girlfriend Dora, and their friends Todd and Elizabeth are on their way to the brothers' uncle Bob's cabin for a camping trip. After stopping off to buy some fireworks, they continue on their way as day turns to night. Matt hits something with the car and stops to investigate. The group hears loud cries of an animal in pain off in the distance, but decide to move onward. After coming across a tree that has been knocked across the road, the group continues on foot and reaches the cabin, only to discover that it is worn down and dilapidated. After finding a wild pig in one of the bedrooms, the group decide to spend the night in the car, where Brian and Matt again hear the loud screaming noises.

The film tells the story of Lucian, the first werewolf born in human form and the first to be called a Lycan. Viktor, a vampire elder, raises the child, and envisions a race of Lycan slaves that could keep guard of the coven's fortress during the day and be used as laborers by the vampires at night. To this end, Viktor demands from the local human nobles a tribute of silver, with which to keep his slaves under control. As Lucian grows up, he and Viktor's daughter Sonja (Rhona Mitra) fall in love and begin a secretive intimate relationship in their adult years. Sonja is reckless and insubordinate, and one night Lucian escapes and rescues Sonja from his werewolf brethren. During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Encouraged by his son Prince Nuada, Balor orders the building of this Golden Army. The humans are devastated by the army. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. In the year 2046, a research facility on Mars is suddenly attacked by an unknown assailant. The film begins deep in the Carpathian Mountains of Romania during the Cold War, where a group of Russian and British explorers are searching for a long-lost 13th-century abbey. As they venture inside and below the church, they make a startling discovery: the abbey is built over the entrance to a vast subterranean cave system. When they try to blast their way in, they cause a landslide that buries the abbey and the men are trapped in the cave below it. Once they recover, they continue on into the cave without a choice in hopes of finding a way out, but as they do, they hear strange sounds coming from the darkness.
