Mantel was born in Montreal, Quebec, Canada. In 1947, on the day his father is buried, Sal Paradise (Sam Riley) is invited by his friend Chad to meet Dean Moriarty (Garrett Hedlund) and Moriarty's 16-year-old wife Marylou (Kristen Stewart). Successful author Mort Rainey (Johnny Depp) suffers a psychotic break when his wife has an affair with a man named Ted (Timothy Hutton). He puts off finalizing the divorce by retreating to his secluded cabin in the rural town of Tashmore Lake in upstate New York. Depressed and suffering from writer's block, Mort is confronted one day by the mysterious John Shooter (John Turturro), a Mississippi dairy farmer who accuses him of plagiarism. Shooter leaves Mort with his manuscript "Sowing Season" which he alleges was copied. Mort dismisses Shooter as a lunatic and throws the manuscript away, but his cleaning lady digs it out of the trash and presents it back to him. With his curiosity piqued, Mort reads the story and is surprised to discover its resemblance to his own story "Secret Window".

Psychiatrist Dr. Miranda Grey (Halle Berry) works at a mental hospital and has a car accident after trying to avoid a girl (Kathleen Mackey) on a road during a stormy night while driving back home. She rushes to try to help the girl. The girl turns out to be a ghost and possesses Miranda's body by burning her after she extends her hand to the girl. Miranda loses consciousness. Dutton) was brutally murdered and that she is the primary suspect. While Miranda copes with her new life in the hospital, the ghost uses her body to carry out messages (most noticeably, she carves the words "not alone" into Miranda's arm), which leads her former colleagues to believe Miranda is suicidal and is inflicting the wounds on herself.

Dr. Frank Linden has a life-size, anatomically correct medical dummy in his office which he calls "Pin". Via ventriloquism, Dr. Linden uses Pin to teach his children, Leon and Ursula about bodily functions and how the body works in a way the children can relate to without it being awkward. Dr. Linden's interactions with the children are otherwise cold and emotionally distant, and his ventriloquism act is the only sign of a more warm and playful side to his nature. Unknown to Dr. Linden, Leon is mentally ill and has come to believe that Pin is alive. Due in part to his mother, who discourages Leon from playing outdoors or bringing anyone home, Leon has no real friends and sees Pin as closest analogue. William Dudley (Leslie Nielsen) is a corrupt mayor of a nameless Midwestern U.S. city who has allowed an oil refinery to be built right in the center of town, far from any river, lake or reservoir. On one typical hot summer day, Herman Stover (Jonathan Welsh), a dangerously disturbed employee at the works has been denied an expected promotion and in addition, finds himself fired. He then decides to take his revenge against the works by opening the valves to the storage vats and their interconnecting pipes, flooding the area and sewers with gasoline and chemicals. It doesn't take long for this act of petty vandalism to start a fire, which starts a chain reaction that causes massive explosions at the refinery, destroying it and spreading a mushroom-cloud of flame that soon engulfs the entire metropolis. The drama focuses on a newly built hospital which, like the refinery and all civic buildings that went up during the mayor's crooked administration, is shoddily built and poorly equipped where the head doctor, Frank Whitman (Barry Newman), and his staff treat thousands of casualties from the fire while the city fire chief Risley (Henry Fonda) keeps in constant contact with the fire companies fighting a losing battle against the fires, and Maggie Grayson (Ava Gardner), an alcoholic reporter, sees it as her chance to make it nationwide with her coverage of the story of the "city on fire".
