The film begins in September 2008 (opening against the backdrop of news of the Islamabad Marriott Hotel bombing) with an elderly Lady Thatcher buying milk unrecognized by other customers and walking back from the shop alone. Over the course of three days, we see her struggle with dementia and with the lack of power that comes with old age, while looking back on defining moments of her personal and professional life, on which she reminisces with her (now-dead) husband, Denis Thatcher, whose death she is unable to fully accept. She is shown as having difficulty distinguishing between the past and present. A theme throughout the film is the personal price that Thatcher has paid for power. Denis is portrayed as somewhat ambivalent about his wife's rise to power, her son Mark lives in South Africa and is shown as having little contact with his mother, and Thatcher's relationship with her daughter Carol is at times strained.

John suffered from epileptic seizures and an autism-like developmental disorder, and the Royal Family tried to shelter him from public view; the script shied away from presenting the Royal Family as unsympathetic, instead showing how much this cost them emotionally (particularly John's mother, Queen Mary). Poliakoff explores the story of John, his relationship with his family and brother Prince George, the political events going on at the time (such as the fall of the House of Romanov in 1917) and the love and devotion of his nanny, Charlotte Bill.

Rather than providing a straightforward biography, the film weaves fantasy sequences with scenes containing somewhat fictionalized accounts of events in Darin's life, and throughout it, the adult singer interacts with his younger self. It chronicles his determination to rise from his working class roots as Walden Robert Cassotto, a frail Bronx boy plagued by multiple bouts of rheumatic fever, and become a singer more famous than Frank Sinatra. To achieve that goal, he forms a band and struggles to find gigs at any nightclub that will hire him.

Several screens of text giving some background are shown before a bust is shown flying over a palace wall and shattering into countless pieces. The film shows the story of a bereaved Queen Victoria (Judi Dench) and her relationship with a Scottish servant, John Brown (Billy Connolly), and the subsequent uproar it provoked. Brown had been a trusted servant of Victoria's then deceased and beloved Prince Consort, Prince Albert; Victoria's chief servants thought Brown might help to ease an inconsolable Queen since the Prince Consort's death in 1861.
