The film picks up immediately after the end of the last film, with Marybeth Dunston (Danielle Harris) blowing off the head of Victor Crowley (Kane Hodder) with a shotgun. She walks away and comes across the bodies of Vernon and John, where she hears Crowley in the woods. Marybeth starts up the chainsaw, but is grabbed by Victor. Marybeth kicks him, and he falls backwards onto the chainsaw, cutting him down the middle. She grabs a shotgun from one of the men and shoots Victor's head, and walks back into the city. She walks into the Jefferson Parish Police Department, immediately having guns drawn on her. Upon discovery of the twenty to thirty bodies at Honey Island Swamp, she is placed as the prime suspect in the murders by Sheriff Fowler (Zach Galligan), even after telling him numerous times about the events of the last two films. The Sheriff heads out to the swamp with the paramedics and fire department, leaving Deputy Winslow (Robert Diago DoQui) in charge of the station until he gets back.

In a flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. She gives him a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, dressed in all white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Fifteen years later, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock by Sheriff Brackett (Brad Dourif), who takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter and Laurie's friend Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's body is loaded into a separate ambulance. When the driver has a traffic accident, Michael awakens and escapes the ambulance, walking toward a vision of his mother dressed in white and leading a white horse.

Stephen and his daughter Justine run from a wireless internet ghost of Michelle, Stephen's late wife. The father and daughter stay at Stephen's cabin, away from the city. A short while after they get there, Stephen's girlfriend Marta arrives. As Stephen and Marta are about to get intimate, a laptop sitting on the table turns on, revealing continuous emails to Stephen from Michelle. As people are enjoying drinks in a bar, a man covered in blood—identified onscreen as "Hero" (Eric Dane)—enters through the door and warns them all of impending danger. No one heeds his warning, so he shows the bar patrons the head of a repulsive creature to make them take him seriously. He is soon pulled through a window and decapitated by one of the monsters. After the carnage, a woman—"Heroine" (Navi Rawat)—bursts through the door and reveals herself to be the recently deceased man's wife. After a brief sentimental moment between the wife and her late husband, the bar patrons begin boarding up the windows in the bar. Despite their efforts, a young monster bursts through an uncovered window and begins attacking. A monster outside bursts its hand through Vet (Anthony "Treach" Criss), Edgy Cat/Jason Mewes has his face torn off and is accidentally shot dead, and the little monster dismembers one of the women—"Harley Mom" (Diane Goldner)—who is initially assumed to have died from massive blood loss.
