A small U.S. special-forces squad travels to Cambodia to follow up on the case of a small medical outpost that has ceased communications, and why the first group sent to the outpost did not report back. The squad soon finds itself confronted by flesh-eating zombies seemingly created by a strange species of scorpion. The squad clears away the zombies, but is then attacked by an infected researcher who attempts to commit suicide with a grenade. Taking place shortly after the original, a professor at the fictional Cuesta Verde University has managed to subdue and contain a "Hyper sapiens" specimen, who is revealed to be an infected survivor from the first film. The professor experiments on the survivor, trying to determine the source of her immortality, with apparent disregard for the rotted state of her body. When he has created a serum that he believes will bring back the dead and grant immortality, he murders one of his students and injects her with the serum. She returns to life, infects the professor, and breaks out of the building.

Amy is an elementary school teacher who suffers from nightmares about doctors and hospitals, stemming from the circumstances of her father's death. On the afternoon of the day he proposed, Amy and her fiancé Nick are in a serious car accident. An ambulance takes Nick away without telling Amy which hospital they are going to. Amy and Lucas, the driver of the truck that hit them, try to find where Nick and Lucas' sister have been taken. Amy begins to hallucinate, seeing disfigured faces, including her own in a mirror. A little girl in her class, Melissa, claims she can help Amy find Nick, and gives Amy the name of St. Rosemary's Hospital. Amy and Lucas learn that St. Rosemary's, which was rumored to be the home of devil worshipers, was destroyed in a fire some 70 years ago, along with all the nurses and staff, who refused to leave. Meanwhile, Nick is having his own strange experiences at St. Rosemary's, such as nurses who spray blood on each other and eat another patient, and a hallway that repeats itself.

Michael Myers has been in a coma for ten years since the explosion at Haddonfield Memorial Hospital and is being transferred to Smith's Grove Sanitarium by ambulance. Upon hearing that he has a niece, Michael awakens, kills the paramedics, and makes his way to Haddonfield to look for her. Dr. Sam Loomis (Donald Pleasence), Michael's former psychiatrist, who also survived the explosion, learns of Michael's escape and gives chase. Angela de Marco (Michelle Pfeiffer) is the wife of mafia up-and-comer Frank "The Cucumber" de Marco (Alec Baldwin), who gets violently dispatched by Mob boss Tony "The Tiger" Russo (Dean Stockwell) when he is discovered in a compromising situation with the latter's mistress Karen (Nancy Travis). Angela wants to escape the mafia scene with her son, but is harassed by Tony who puts the moves on her at Frank's funeral. This clinch earns her the suspicion of FBI agents Mike Downey (Matthew Modine) and Ed Benitez (Oliver Platt), and also of Tony's wife Connie (Mercedes Ruehl), who repeatedly confronts Angela with accusations of stealing her husband. To further complicate things, Mike Downey is assigned to monitor all of Angela's movements as part of an undercover surveillance operation, but cannot resist becoming romantically involved with Angela himself. Angela's attempts to break away from the Mob result in comic mayhem and a climactic showdown in a honeymoon suite in Miami.

Without the corporate or private financial resources of more well-established teams, the Specials often get underrated villains, small disasters, and the occasional alien invasion to repel — all of which are deemed too low-priority for other superhero teams. Even the members with formidable powers have shortcomings that prevented a transition for the team as a whole.
