An unknown viral outbreak has supposedly devastated the world. A family of three; Ray (Alexander Skarsgård), Claire (Andrea Riseborough) and their daughter Zoe (Emily Alyn Lind) have taken refuge in an abandoned fallout shelter shortly after the catastrophe to hide from the monsters outside. Zoe wakes one night after having a nightmare of the supposed monsters she calls 'Breathers' and believes the shelter is in danger. Ray and Claire convince Zoe that they are safe and there is nothing to fear. Zoe also talks to her doll, Olive, for comfort and support and sees Olive as a real person. The family have difficulty coping in their cramped environment but manage by having a set of rules, such as always keeping calm and never leaving the shelter. The three soon discover at dinner that their already dwindling rations are also being consumed by a rat and try to find and kill it. The family also discover that the rat managed to sneak into the shelter through another opening other than the main barricaded hatch. They find a small air duct but Ray reassures Zoe that the breathers wouldn't be able to fit through the duct. It is revealed that the they have taken shelter for 301 days and that Ray believes they're most likely the only ones left alive. Claire talks to Ray about her concern over their food supplies but Ray assures her everything will be fine and that their main priority is to keep Zoe alive, regardless of what happens to them. Ray and Claire then have sex and Ray falls asleep.

In 2013, Earth's coastal cities come under attack from Kaijus, colossal beasts that periodically emerge from a dimensional portal on the Pacific Ocean floor called "The Breach." To fight them, the Pacific Rim nations build the Jaegers, equally colossal humanoid war machines. Each Jaeger is controlled by two or more pilots whose brains are linked, called "drifting" to share the heavy mental load of operating the machine. The Jaegers are initially effective, but many are destroyed as the Kaijus grow more powerful and their attacks become more frequent. In 2025, the participating governments deem the Jaeger program unsustainable and discontinue it in favor of building defensive coastal walls. The four remaining Jaegers are redeployed to Hong Kong to protect the coast until the walls are completed. Jaeger commander Stacker Pentecost resolves to make a last-ditch attempt to end the Kaiju threat by destroying The Breach with a nuclear weapon, even though past nuclear strikes on the portal have been unsuccessful.

In August 1987, new parents Joy White (Shepherd) and Carl Tyson (Cross) took their 19-day-old daughter Carlina to Harlem Hospital in New York with a high fever. Sydney Wells is a successful classical violinist from Los Angeles who has been blind since she was five years old, caused by an accident with firecrackers. Fifteen years later, after celebrating conductor and pianist Simon McCullogh's birthday during rehearsal, Sydney undergoes a cornea transplant, which causes her eyesight to return, a bit blurry at first. As time goes on, Sydney's vision begins to clear; however, she also begins experiencing terrifying visions, mostly of fire and of people dying. She also sees people that are already dead, on one occasion when a girl passes right through her. Sydney attempts to unravel the mystery of the visions, and also to convince others, primarily her visual therapist and fellow violinist, Paul Faulkner, who helps her in her quest. She knows that she is not going insane.
