The Expendables—led by Barney Ross and formed by Lee Christmas, Gunnar Jensen, and Toll Road—extract former member Doctor Death, a knives specialist and team medic, from a military prison during his transfer on a train. They recruit Doc to assist them in intercepting a shipment of bombs meant to be delivered to a warlord in Somalia. Arriving there, they reunite with Hale Caesar, who directs them to the drop point, where Ross is surprised to find out that the arms trader providing the bombs is Conrad Stonebanks, a former co-founder of the Expendables who went rogue and was presumed dead. In the ensuing firefight, The Expendables manage to kill all but Stonebanks, who shoots Caesar twice. As the team attempts to aid him, they are forced to retreat due to Stonebanks' advanced weaponry, and Caesar is severely injured in the process.

In the Southeast Asian nation of Shadaloo City, civil war has erupted between the forces of crazed military dictator General M. Bison (Raul Julia) and the Allied Nations, a multinational military force, led by their regional commander, Colonel William F. Guile. Bison has recently captured several A.N. relief workers, and via a live two-way radio broadcast, demands Guile secure a US$20 billion ransom in three days or else face responsibility for the hostages' deaths. Guile refuses and vows to track Bison down, but his assistant, Sergeant Cammy (Kylie Minogue) is only partially able to pinpoint Bison's location to the river-delta region outside the city. One of the hostages is Guile's close friend, Carlos "Charlie" Blanka (Robert Mammone), who Bison orders taken to his lab for his captive witch doctor, Dhalsim (Roshan Seth), to turn into the first of his supersoldiers. Though Charlie is severely disfigured by the procedure, Dhalsim secretly alters his cerebral programming to maintain Charlie's humanity.

In 1969, an American US Army Special Forces team receives orders to secure a village against North Vietnamese forces. Private Luc Deveraux (Jean-Claude Van Damme) discovers members of his squad and various villagers dead, all with their ears removed. Deveraux finds Sergeant Andrew Scott (Dolph Lundgren), who has gone insane, with a string of severed ears and holding a young boy and girl hostage. Devereaux, who is near the end of his tour of duty, tries to reason with Scott, who shoots the boy and orders Devereaux to shoot the girl to prove his loyalty. Deveraux refuses and tries to save the girl, but Scott kills her with a grenade. The two soldiers shoot each other to death. Deveraux and Scott's corpses are recovered and iced by a second U.S. Special Forces squad, their deaths covered up as "missing in action.

Lyon Gaultier is a paratrooper in the French Foreign Legion, stationed in Djibouti, North Africa. Lyon deserts his legion when they withhold letters from his brother's wife and ultimately refuse to let him see his brother.
