Eight years after Harvey Dent's (Aaron Eckhart) death, the Dent Act grants the Gotham City Police Department powers which nearly eradicate organized crime. Feeling guilty for covering up Dent's crimes, Police Commissioner James Gordon (Gary Oldman) writes a resignation speech confessing the truth but decides not to use it. Batman has disappeared, and Bruce Wayne (Christian Bale) has become a recluse. Cat burglar Selina Kyle (Anne Hathaway) obtains Bruce's fingerprints from his home, kidnaps a congressman, then disappears. Selina hands Bruce's fingerprints to Phillip Stryver (Burn Gorman), an assistant to Bruce's business rival John Daggett (Ben Mendelsohn), in hope of having her criminal record erased. Stryver double-crosses Selina, but she uses the congressman's stolen phone to alert the police to their location. Gordon and the police arrive to find the congressman, and then pursue Stryver's men into the sewers while Selina flees. A masked militant named Bane (Tom Hardy) captures Gordon. Gordon escapes and is found by John Blake (Joseph Gordon-Levitt), a once-orphaned patrol officer who has deduced Batman's true identity from their similar backgrounds. Gordon promotes Blake to detective, with Blake reporting directly to him.

At St. John's Orphanage in 1882, children, including a young Albert Fish, are being paddled as punishment for their sins. Albert Fish as an adult (Patrick Bauchau) then tells an anecdote of a horse that some older boys at the orphanage set on fire, comparing himself to the horse. He regularly whips himself with a belt while hallucinating himself as he appeared in the orphanage. Fish kills a boy scout, Francis McDonnell, before visiting the Budd family home, where he abducts and murders ten-year-old Grace Budd (Lexi Ainsworth) on June 3, 1928 under the pretense of taking her to his niece's birthday party. Throughout the film is a film noir-style narration by Detective William King (Jack Conley), of the Missing Persons Bureau. King searches for Grace Budd for six years, before finally tracking down and arresting Fish. Fish is found guilty despite evidence of his insanity, and promptly sentenced to die in the electric chair.

In 1989, Josey Aimes returns to her hometown in northern Minnesota with her children, Sammy and Karen, after escaping from her abusive husband. She moves in with her parents, Alice and Hank. Following the murder of Diana Floss (Careena Melia) in a restaurant shooting in Cape Ann, Massachusetts in 1973, her fiancé Joe Nast (Jake Gyllenhaal) elects to stay with her parents. Her father Ben Floss (Dustin Hoffman) is a realtor whose business partner has recently left. Ben and Joe go into business as Floss & Son, as this was their plan before Diana's death. Joe goes to the post office to retrieve all the invitations that had been sent out for his and Diana's wedding, and with the help of Bertie Knox (Ellen Pompeo) he retrieves seventy four of seventy five. She finds the last invitation and takes it to his house later that night. He drops her off at a local bar, and returns home, despite her inviting him in for a drink. Joe and Ben attend a local property fair, and Ben pitches the idea of redeveloping a block in the town to developer Mike Mulcahey (Dabney Coleman). Mulcahey agrees, but they need to get all the tenants to agree.
