John (Scott Adkins) awakens from a coma to find that his wife and daughter were brutally murdered in a home invasion. Haunted by images of the attack, he vows to kill the man responsible: Luc Deveraux (Jean-Claude Van Damme), one of the original Universal Soldiers (UniSol). While John tries to piece his reality back together, things are complicated by a relentless UniSol named Magnus (Andrei "The Pit Bull" Arlovski), who is determined to kill John. Luc has emerged operating the Unisol Church of Eventualism, taking in wayward Unisols whom the government has been secretly operating as remote controlled sleeper agents. His mission is to liberate these Unisols from the implanted memories and the lies the government has inserted in them. As John gets closer to Deveraux and the rogue army of genetically enhanced warriors, he discovers more about himself and begins to call into question everything he believed to be true.

St. Jude Square is a neighborhood living in fear and despair. U.S. forces join up with the Ukrainian army at the plant, but quickly retreat when the NGU slaughters most of them effortlessly. Dr. Porter (Garry Cooper), Dr. Colin's former colleague on the Universal Soldier program, revives four UniSols to take down the NGU, but they are systematically eliminated.

The film opens on a close-up of a frightened woman's eyes. The camera pulls back to reveal that she's undergoing a blind tasting of coffee by local TV reporter CJ Nicholas (Jesse Metcalfe). He heads to the courthouse to watch Shreveport District Attorney Mark Hunter (Michael Douglas) give his final argument on a murder case. CJ pigeonholes Assistant D.A. Ella Crystal (Amber Tamblyn) afterward and asks her for the videotape of the murderer's interrogation. He also presses her for a date. She reluctantly agrees to both.

Carol Hunnicut (Anne Archer) goes out on a blind date set up by her friend, in a hotel restaurant in Los Angeles with Michael Tarlow (J. T. Walsh), a lawyer about whom she knows very little. When Tarlow receives a message during dinner to call a client, she accompanies him to his hotel suite while he makes the call. While she is in the bathroom in the suite, the client, gangster Leo Watts (Harris Yulin), arrives in person with one of his hit men, Jack Wootton (Nigel Bennett). Watts accuses Tarlow of stealing money from him. After a fearful Tarlow admits that he did, Wootton shoots him to death. Carol witnesses Tarlow's admission that he stole the money and the murder itself from an adjacent room, but Watts and Wootton do not detect her. After they leave, she waits for almost an hour before fleeing the scene. Fearing for her life, Carol does not report the murder to the authorities.
