The film opens with fast-talking businessman and diamond dealer Kyle Miller (Nicolas Cage) speaking to a client over the phone as he returns to his lavish mansion home, his wife Sarah (Nicole Kidman) and his teenage daughter Avery (Liana Liberato). Despite the charming facade, it is immediately clear that the family is dysfunctional and emotionally distant: Avery disrespects her parents and, despite being forbidden to do so, sneaks out of the house to go to a party with her friend Kendra. Sarah appears bored with life as a housewife and yearns for more in her marriage while Kyle seems to harbor a hidden aversion towards his wife. Just as Kyle is about to leave for a business transaction, the house is suddenly invaded by a gang of robbers masquerading as police.

The prequel is set in Mexico in the early 1900s and begins with an American author, Ambrose Bierce, experiencing a nightmare in which he dies at the hands of Pancho Villa. Bierce wakes and talks to a local bartender about his intentions to join Pancho Villa's revolutionary army. They later rob Bierce's stagecoach because of Reece's belief that Bierce possesses an invaluable object. The object turns out to be the manuscript for Bierce's new book. Annoyed by this, Madrid leaves Reece to die in the desert. She is found by the posse who use her to track the two.

The 1952 segment deals with Claire Donnelly (Demi Moore), a widowed nurse living in suburban Chicago, who becomes pregnant by her brother-in-law (Jason London) and decides to undergo abortion in order not to hurt her late husband's family. However, abortion at the time is strictly illegal. Claire eventually finds another nurse (CCH Pounder) who provides her the phone number of a woman who can find her someone to perform the abortion. The woman on the phone tells Claire that the only trustworthy abortionist she knows is located in Puerto Rico, and Claire cannot afford the travel costs. After a failed attempt to end her pregnancy with a knitting needle, Claire eventually contacts a man who comes to her home and performs a clandestine procedure on her while she lies on top of a kitchen table. Claire finally manages to abort, but dies shortly afterwards due to hemorrhage.
