In the peaceful Appalachian hills of eastern Kentucky, toxins are being dumped into abandoned mines, causing environmental havoc, but the locals, mindful of their jobs and the power of the mine owners, can do nothing. EPA CID agent Jack Taggart (Steven Seagal) is sent to investigate, after a fellow agent is found dead, probably not by accident. The EPA has received an anonymous letter from the town of Jackson, Kentucky, and Taggart goes there undercover to continue his colleague's investigations.

Washed up former race driver Brent Magna (Ethan Hawke) arrives home to find his house ransacked and his wife, Leanne, missing. He then receives a phone call from a mysterious man known only as The Voice (Jon Voight), who reveals himself as the kidnapper of Magna's wife. Macy stars as the COB (chief of the boat) on a fictitious US Navy submarine, the USS Swordfish (although there were two actual submarines named USS Swordfish that served the US Navy during and after World War II, neither are represented by the sub in this film). While on patrol in the North Atlantic, the ship is ambushed by two German U-boats. Swordfish destroys one of them, but is critically damaged by a torpedo fired from the second one, the fictitious U-boat U-429 (again, there was a real WWII German submarine U-429, not actually represented in the film). Eight members of the American crew escape and are held prisoner on board U-429. However, the American executive officer had previously contracted meningitis and given it to his captain, who brought it aboard the U-429. Within days, over half the German crew, as well as some of the Americans, contract the disease and die. Although the Germans and Americans begin to work together to keep the boat operational, not all the U-boat crew is willing to go along with this arrangement. Also, another U-boat and an American destroyer (the fictional USS Logan, not related to the actual attack transport of that name) threaten to overwhelm their tentative truce.

Casey Ryback has retired from the United States Navy and is a chef at the Mile High Cafe in Denver, Colorado. Casey is taking his estranged niece Sarah to Los Angeles to visit the grave of Sarah's father. Sarah and Casey board the Grand Continental, a train traveling from Denver to Los Angeles through the Rocky Mountains. Aegis Oil operates Aegis 1, an oil refinery and several oil rigs in Alaska. They purchased the oil rights from the local Alaskan Natives 20 years ago, but will lose them if the refinery isn’t on-line by a certain deadline. With 13 days to go, and billions of dollars at stake, the company cuts corners and uses faulty equipment. Hugh Palmer, a rig foreman, is aware of this; as he predicts, his rig catches fire. It takes Forrest Taft (Seagal), a specialist in dealing with oil drilling-related fires, to extinguish the fire. Taft refuses to believe Hugh’s story of faulty equipment, but discovers that it’s true after accessing the company’s computer records. Michael Jennings (Michael Caine), the ruthless CEO of Aegis, believes that Hugh's carelessness is to blame and arranges for him to be ‘dealt with’ by his henchmen MacGruder (John C. McGinley) and Otto (Sven-Ole Thorsen).
