The film follows 18 individuals in separate but sometimes related groups on the campus of a Japanese university hospital. These groups include a mother and father of an unborn child; a group of friends during a dance rehearsal; staff at the university hospital and a team of students researching urban legends. Ostensibly, the plot is concerned with a mysterious outbreak resembling a viral pandemic which causes the film's protagonists to suddenly die one-by-one. It is suggested during the film that this is due to secret experiments being conducted by the United States Armed Forces in the hospital. However, rather than attempting to resolve the mystery of the outbreak, the focus of the film is largely devoted to the relationships between the characters and their reactions to their own and each other's impending deaths. Ishii has stated the film is made to show the absurdity of the characters not knowing when or why their deaths will occur and highlights the importance of drama and dialogue within the film, in contrast to his previous works.

8 months after triumphing over Serizawa Tamao (Takayuki Yamada), Takiya Genji (Shun Oguri) still struggles to attain supremacy at Suzuran All-Boys High School. Following a decisive defeat at the hands of the legendary Rindaman, and on the verge of graduating without fulfilling his goal, Genji grows quietly desperate. He begins challenging Rindaman regularly, but consistently fails to beat him. His situation escalates when he unwittingly breaks a non-aggression pact between Suzuran and a rival school, Housen Academy, by coming to the aid of Kawanishi Noboru (Shinnosuke Abe) during a heated confrontation. Genji learns that the agreement between the two schools was established two years prior when, during a skirmish, Noboru violated a gang law and used a weapon to fatally wound Housen's former leader, Bitō Makio. Suzuran had subsequently sworn not to interfere with Housen's retribution upon Noboru's release from prison. Genji's protection of Kawanishi provokes Housen's current leader, Narumi Taiga (Nobuaki Kaneko), to declare war against Suzuran. Genji and his allies go on the defensive, engaging in several violent conflicts with Housen's "Army of Killers".

In 1975, an obscure rock band called Gekirin record a song called "Fish Story" that anticipates the sound of punk rock that's just beginning in New York and London. The band breaks up shortly after "Fish Story" is released due to record company interference, but the song has an ongoing life over the years. The film jumps between decades as the recording plays a role in several seemingly unrelated scenarios: a member of the hospitality staff on a ferry (Mirai Moriyama) wins the heart of a beautiful girl while battling hijackers; Masaru (Gaku Hamada) and his buddies look for women on a lonely and boring evening; a comet heads towards Earth while a strange man in a record store lectures those around him about the impending destruction of the Earth; and how Gekirin came to write the song.
