Kurt Sloane (Jean-Claude Van Damme) is the cornerman for his brother Eric (Dennis Alexio), an American kickboxing champion. Kurt and Eric travel to Bangkok to challenge Tong Po (Michel Qissi), Thailand's undefeated champion. After a brutal first round, Kurt realizes that they are outmatched by Tong Po and tries to convince his brother to stop the fight. Unwilling to give up, Eric returns for the second round and continues to take a vicious beating, finally prompting Kurt to throw in the towel. Tong Po ignores him and continues the assault, which ultimately leaves Eric paralyzed from the waist down. Eric is then carried outside and left on the street without any help, until Winston Taylor (Haskell Anderson), a retired US Army special forces member, intervenes and agrees to take the brothers to the hospital. After several hours of surgery, the doctor informs Kurt that Eric will never walk again.

It is one year after the events of the first film. It is revealed that kickboxing brothers Kurt and Eric Sloane have been killed by Tong Po in Thailand. David Sloane (Mitchell), the youngest and last surviving brother, struggles to keep the family kickboxing gym afloat. Although his will to compete has waned since the loss of his brothers, financial problems eventually force Sloane to fight again in a new organization run by a crooked promoter. His surprising comeback ultimately attracts the attention of Po (Qissi) who, having been defeated by Kurt, seeks to defeat David in the ring and regain his honor. But when Sloane announces his retirement after the bout, Po's manager Sanga (Tagawa) hires a group of thugs to burn down the gym, injuring Sloane and killing one of his young students.

U.S. Army Captain Frank Dux, trained from his youth in the ways of Ninjutsu by Senzo Tanaka, honors his mentor by taking the place of Tanaka's deceased son Shingo in the illegal Kumite martial-arts tournament. During his childhood, Dux and a group of friends invaded Tanaka's home to steal a katana, but he was apprehended by Senzo and Shingo while attempting to return the katana to its display rack. Impressed by Dux's honesty and lack of fear, Senzo had him train with Shingo in martial science. Following Shingo's death, Senzo agreed to train Dux as a member of the Tanaka clan. When Dux's superiors learn of his invitation to the Kumite, they refuse to let him go; in return, he goes AWOL, says goodbye to his mentor and leaves for Hong Kong. Two CID officers, Helmer and Rawlins, are summoned to track down and arrest Dux for desertion.
