In the summer of 1982, Sam Brenner and Will Cooper are thirteen-year-olds who love playing in the local arcade. Brenner enters an arcade game world championships. At the event, the MC tells that a time capsule will be launched into space containing a videocassette that includes scenes from the championships. Set in Atlanta, the film opens with fitness trainer Jules Baxter (Cameron Diaz) and her dance partner Evan Webber (Matthew Morrison) performing on the TV show Celebrity Dance Factor. They are crowned the winners of the show, but Jules vomits in their trophy, discovering that she is pregnant. Jules struggles when trying to balance her pregnancy with her normal active life. A year after the attack on Lake Victoria by prehistoric piranhas, an eradication campaign has left the lake uninhabitable, and the town itself has been largely abandoned as a result of the drying-up of the Lake. Meanwhile at Cross Lake, two farmers (Gary Busey and Clu Gulager) search the waters to recover the body of a dead cow. Piranha eggs that have been laid inside the cow hatch, and the farmers are killed by the swarm, but before they both die, one farmer pops out of the water, takes a piranha, bites it, and then spits it up before sinking back into the water.

Over-worked, bullied and terrified of being put in foster care, seventeen-year-old Katie Gibbs (Lucy Hale) is a singer who does her stepmother and stepsiblings' bidding with no complaints. In the town of Ogden Marsh, Iowa, David Dutten (Timothy Olyphant), the local sheriff is enjoying a baseball game. His deputy, Russell Clank (Joe Anderson), spots Rory Hamill, a local resident (and the town drunk who's has been sober for two years), entering the outfield holding a shotgun. David, knowing his history, and owing to his behaviour, believes he has started drinking again. David is forced to shoot and kill the unresponsive Rory when he raised his weapon and aimed it at the Sheriff. David and his wife Judy (Radha Mitchell), the community doctor, begin to notice other town residents exhibiting bizarre behavior, including listlessness and repetitive speech. The next night, a local farmer burns his house down with his wife and son trapped inside, killing them. By the time firefighters and other townspeople arrive, he is found mowing his lawn while his house burns.

In a flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. She gives him a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, dressed in all white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Fifteen years later, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock by Sheriff Brackett (Brad Dourif), who takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter and Laurie's friend Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's body is loaded into a separate ambulance. When the driver has a traffic accident, Michael awakens and escapes the ambulance, walking toward a vision of his mother dressed in white and leading a white horse.
