A suicidal Hunter "Patch" Adams (Robin Williams) commits himself into a mental institution. Once there, he finds that using humor to help his fellow patients gives him a purpose in life. Because of this he wants to become a medical doctor and two years later enrolls at the Medical College of Virginia (now known as VCU School of Medicine) as the oldest first year student. He questions the school's soulless approach to medical care and clashes with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients as patients and not bond with them as people. Because of this and incidents such as setting up a giant pair of legs during an obstetric conference, he is expelled from the medical school, although he is later reinstated due to his methods actually helping patients improve. Adams encourages medical students to work closely with nurses, learn interviewing skills early, and argues that death should be treated with dignity and sometimes even humor.

In 1952, Ed Wood (Johnny Depp) is struggling to join the film industry. Upon hearing of an announcement in Variety magazine that producer George Weiss (Mike Starr) is trying to purchase Christine Jorgensen's life story, Ed is inspired to meet Weiss in person. Weiss explains that Variety‍ 's announcement was a news leak, and it is impossible to purchase Jorgensen's rights. The producer decides to "fictionalize" the film, titled I Changed My Sex!. Ed tries to convince Weiss that he is perfect to direct I Changed My Sex! because he is a transvestite, but is unsuccessful since Weiss wants a director with experience. One day, Ed meets his longtime idol Bela Lugosi (Martin Landau), after spotting him trying out a casket. Ed drives Bela home and the two become friends. Later, Wood persuades Weiss to let him direct I Changed My Sex! by convincing him that having a star in the film would sell tickets, and they could sign Bela for a low price.

An unemployed drifter named John Nada (Roddy Piper) finds construction work in Los Angeles, and befriends fellow construction worker Frank Armitage (Keith David), who leads him to a local shantytown soup kitchen. There, Nada encounters strange activity around the church: a blind preacher (Raymond St. Jacques) loudly chastising others to wake up, a police helicopter scouts them overhead, and a drifter (George Buck Flower) complains that his TV signal is continually interrupted by a man warning everyone about those in power. Nada discovers the nearby church is a front; the choir heard outside is an audio recording and the building is filled with scientific equipment and cardboard boxes. Nada finds a box hidden in the wall, but flees when the preacher notices him. That night, the police attack and bulldoze the shantytown. Nada returns in the morning to find the church empty, but with the hidden boxes still in the wall. He takes one of the boxes and in an alley, he opens the box and finds it filled with sunglasses. Taking a pair, he hides the box in a garbage can.

Going about his day, Toddy, a performer at Chez Lui in Paris, sees Labisse, the club owner, auditioning a frail, impoverished soprano, Victoria Grant (Julie Andrews). After the audition, Labisse drily writes her off, and she responds by sustaining a pitch to shatter his wine glass using resonant frequency. That night, Richard comes to Chez Lui as part of a straight foursome and Toddy incites a brawl by insulting Richard and the women in his group. Labisse fires Toddy and bans him from the club. Walking home, he spots Victoria dining at a restaurant, and she invites him to join her. The Cameron family seems, on the surface, to be the perfect family, but things are not as they seem. Their two teenage children, Scott and Sandy, fall in with the wrong crowds at their high school and eventually become involved with drug experimentation. Sandy, after ingesting angel dust made by her boyfriend in the school's chemistry lab, jumps through a glass window of the school (purposely cutting her arms with the cut glass in the process) and is subsequently paralyzed from the fall. A caring guidance counselor, Eileen Phillips, sees the problem that is going on in the school and, after other tragic incidents involving two other students (one of which involves Scott and his girlfriend smoking drugs and crashing their car off a cliff) and when no one else on the staff is willing to do anything about it, takes the steps to deal with and confront the problem. The aftermath of this tragedy makes Scott and Sandy's parents realize that even their "perfect" kids can be affected by drugs, especially after Scott has a violent reaction and goes into the hospital. At a school assembly, Eileen Phillips storms in and confronts the students about the increasing drug use, sending a message to the crowd about the effects of what the drug problem is doing to the kids and the tragedies that resulted because of it.
