In 2012, private corporations own and manage the prison systems. Getaway driver Carl "Luke" Lucas (Luke Goss) is arrested after a robbery for his crime boss Markus Kane (Sean Bean) goes wrong. As his accomplices are robbing the bank, two officers casually enter the building. Luke tells them to abort, but they refuse; Luke intervenes, but it results in the death of one of the three accomplices. Luke shoots and kills one of the officers and dumps off his accomplices in order to fulfill Markus's wishes. In doing so, Luke is eventually captured by the police following a high-speed chase and sentenced to serve time on Terminal Island.

Sydney Wells is a successful classical violinist from Los Angeles who has been blind since she was five years old, caused by an accident with firecrackers. Fifteen years later, after celebrating conductor and pianist Simon McCullogh's birthday during rehearsal, Sydney undergoes a cornea transplant, which causes her eyesight to return, a bit blurry at first. As time goes on, Sydney's vision begins to clear; however, she also begins experiencing terrifying visions, mostly of fire and of people dying. She also sees people that are already dead, on one occasion when a girl passes right through her. Sydney attempts to unravel the mystery of the visions, and also to convince others, primarily her visual therapist and fellow violinist, Paul Faulkner, who helps her in her quest. She knows that she is not going insane.

The story is set during the Great Depression, specifically around the time of the 1933 Long Beach earthquake. Camilla (Salma Hayek) is a fiery, beautiful Mexican café waitress who aspires one day to be above her current standing in society, to make something of herself, to give her and her future children a place and chance in the world. Farrell's character is portrayed as a struggling writer who comes to Los Angeles's Bunker Hill area to start his writing career. Though he falls in love with Camilla, he does not marry her. Later, Camilla is infected by tuberculosis and leaves Arturo without informing him. When Arturo finds her, she is about to die and he promises to marry her, but Camilla dies and Arturo writes a novel dedicated to Camilla. Arturo writes a dedication in one of his books to her and throws it into the sand.

A man is seated in a diner who is revealed as Harold, a traveling salesman. He is joined by a gentleman who makes him uncomfortable. After leaving the diner, Harold is found dead, which leads to FBI Agents Thomas Mackelway (Aaron Eckhart) and Fran Kulok (Carrie-Anne Moss) being brought in on the case. There appears to be history between Mackelway and Kulok and a cloud of competency is addressed by flashbacks of Mackelway's recent suspension and issues he's undergone. In 1876, U.S. Army captain Civil War veteran Nathan Algren is traumatized by his participation in atrocities during the Indian Wars, and has become an embittered alcoholic. Algren is approached by his former commanding officer, Colonel Bagley, on behalf of Japanese businessman Omura, who wishes to hire distinguished U.S. soldiers to train the Imperial Japanese Army to suppress a samurai rebellion, led by Katsumoto Moritsugu. In exchange, Japan would ratify a lucrative trade agreement that would grant the U.S. exclusive rights to supply arms to the Japanese government. Although Algren despises Bagley for having ordered atrocities during the Indian wars, he accepts the job for the money and sails to Japan. Most of the soldiers being trained are little more than inexperienced peasants and farmers.The training is interrupted when the samurai rebels attack a railroad owned by Omura; Bagley orders the regiment to mobilize, overruling Algren's objection that the soldiers are not ready. Algren is proved correct: during a battle in a foggy forest in Yoshino Province, the undisciplined and intimidated soldiers panic and are quickly routed by the samurai horsemen. Algren kills several samurai with his revolver and saber but is eventually knocked from his horse. Algren desperately fights using a spear tipped flagstaff with a flag of a white tiger until he collapses from complete exhaustion. As he is about to be vanquished, he kills his would-be executioner with the staff's spear tip. His ferocity and refusal to surrender, as well as the image of the white tiger on the flag he is fighting with, reminds Katsumoto of a recurring dream. As a result, Algren is spared and taken captive to the rebels' village in the mountains.
