The film is based on the story of Erzsébet Bathory, a Hungarian countess in the 16th and 17th centuries. Her story takes place in a part of the Kingdom of Hungary that is now Slovakia. In this retelling, the Countess is a healer who conducts medical experiments and rudimentary autopsies in a "hospital" beneath her castle. She forms a relationship with a reputed witch, Darvulia, who saves her from poisoning. The witch promises Erzsebet a son and eternal beauty. In return, Erzsebet must sacrifice both love and her reputation. Darvulia becomes Erzsebet's companion. Meanwhile, maidens in the area have been dying of seemingly unrelated causes, and Erzsebet is seen bathing in a large tub of red liquid as the girls' now-mutilated corpses are buried nearby. Two monks later conclude that the water is not blood but is simply colored red by herbs.

In 2004, a satellite detects a mysterious heat bloom beneath Bouvetøya, an island about one thousand miles off the coast of Antarctica. The film begins in Hudson, Ohio, where Scott "Scotty" Thomas (Scott Mechlowicz) gets dumped by his girlfriend, Fiona (Kristin Kreuk), immediately after his high school graduation in 2004. With his best friend, Cooper Harris (Jacob Pitts), Scott attends a graduation party that evening and listens to a performance of the film's main theme song "Scotty Doesn't Know", that crudely details the affair Fiona was having with the band's singer, Donny (Matt Damon), while she was with Scott. Drunk and angry, Scott returns home and receives an e-mail reply from his German pen pal, Mieke (Jessica Boehrs), who expresses sympathy towards Scott after Fiona breaks up with him, and suggests they arrange to meet in person. However, the entire time Scott has known her he has mispronounced Mieke as "Mike", thinking that she is male. After Cooper suggests that "Mike" may be a sexual predator, Scott angrily tells Mieke to stay away from him. Scott's younger brother, Bert (Nial Iskhakov), informs him that "Mieke" is a common girl's name among Germans. Realizing his mistake, and that he has feelings for Mieke, Scott tries desperately to contact her again, only to find out that Mieke has blocked his email address. Encouraged by Cooper and with him in tow, Scott decides to travel to Europe, seek out Mieke, and apologize to her face-to-face.

An NSA mission to collect intel on Anarchy 99, a mercenary group made up of former Russian soldiers goes awry when the agent's identity is discovered by the group. NSA Agent Augustus Gibbons, overseeing the operation to get any intel on the group's plans for the biochemical weapon named "Silent Night" that has been missing since the fall of the Soviet Union, believes the only way to get close is to recruit an agent that would not have any ties to the United States government. He selects Xander Cage, also known as X, an extreme sports professional and host of his own television show who is outspoken against the government and was recently captured by the FBI for stealing and destroying a prominent California senator's car as an act of protest. Gibbons puts Cage through two tests - stopping a staged diner robbery, and escaping from a drug cartel's plantation in Colombia - and offers Cage the mission. Cage reluctantly agrees when Gibbons offers to wipe his criminal record away.

When a mission to retrieve a stolen suitcase bomb goes bad, Central Intelligence Agency (CIA) agent Kevin Pope (Rock) is killed. L'Ultime Sacrifice raconte l'histoire vraie du héros de guerre William H. Pitsenbarger (Jeremy Irvine), surnommé PJ, un sauveteur en hélicoptère de l'armée américaine qui a sauvé plus de soixante hommes à lui seul. Lors d'une mission, le 11 avril 1966, on lui propose de s'échapper avec le dernier hélicoptère quittant la zone de combat en plein tir, mais il choisit de rester et de sauver les vies de ses camarades de la première infanterie, avant de faire l'ultime sacrifice lors d'une des plus sanglantes batailles de la guerre. Trente-deux ans plus tard, un employé respecté du Pentagone, Scott Huffman (Sebastian Stan) est missionné pour étudier la demande faite par le meilleur ami de PJ (William Hurt), et ses parents (Christopher Plummer & Diane Ladd), d'attribuer à PJ la médaille d'honneur du Congrès, à titre posthume. Huffman cherche donc les témoignages de vétérans qui ont assisté au sauvetage entrepris par Pitsenbarger, comprenant ceux de Takoda (Samuel L. Jackson), Burr (Peter Fonda) et Mott (Ed Harris). Pourtant, alors qu'Huffman en apprend plus sur les actes héroïques de PJ, il se rend compte que des personnes haut placées essaient de cacher ce qu'il s'est réellement passé et ont refusé la demande de médaille depuis des années. Huffman fera tout pour obtenir justice pour PJ, quitte à y perdre sa carrière.
