A young pizza delivery boy named Keno inadvertently encounters burglars on his route and tries to stop them. Seeing him as a witness, the burglars attack Keno, who proves to be an expert martial artist, but he is soon overwhelmed before the arrival of the Ninja Turtles. As a crime wave rises in New York City, Channel 3 news reporter April O'Neil talks about rumors of young children and teenagers being reported as the majority of the thieves, connecting it to a rumored Foot Clan, which originated in Japan. On an ordinary night, she comes across thieves stealing from a news van and is attacked. She is rescued and one of her rescuers leaves behind a sai as police arrive to collect the beaten criminals. In the storm sewers, four anthropomorphic turtles return to their underground home and meet with their master Splinter, who advises them to remain hidden from the world and work only in the shadow. Raphael admits he lost his weapon, and when the others order pizza he leaves in disguise to see a movie. After leaving the movie, he encounters Casey Jones who tries to exact justice on a pair of muggers by beating them with sporting implements. After a brief stand off between the two, Casey takes off into the city and Raphael loses his temper. Returning home, Splinter confronts Raph about his temper.

The film takes place entirely in one evening, with the time being indicated chronologically on the clock in Johnny's Trans Am. Early in the evening Johnny meets with his friend Lippy, an eccentric cocaine dealer played by Michael Winslow. They discuss the planned burning of an apartment building, called for by the mob. As their discussion progresses, it becomes clear that it is Johnny who must carry out the burning of the building before the night is over, and moreover, his mother and sister live in the targeted building. This request from the Mob pushes Johnny to plan a split from the Mob, which proves difficult. By the end of the night Johnny must save his sister and mother from the burning of their building, and also rescue his girlfriend and their newborn child from the clutches of the Mob.

Eddie Marino (Robert Forster) is a factory worker in New York City. He has a wife named Vickie (Rutanya Alda) and an eight-year-old son named Scott. Eddie's friend and co-worker, Nick (Fred Williamson), and two other co-workers, Burke and Ramon, have formed a secret vigilante group because Nick and the group are fed up with the crime in their neighborhoods. Nick and his group are also tired of the police, because the police always fail to protect people. Nick's "group" has support of various residents of the neighborhood who indirectly help them. In one example, a local thug stalks and chases a young woman to a rooftop of an apartment building where the thug robs and then kills her. An old lady who witnesses the thug says nothing to the police, but points out the thug to Nick and his group the next day. Nick and his friends forcibly grab the thug off the streets and place him in their van and drive away. The thug is later found dead in a vacant lot with all of his arms and legs broken and his head bashed in.

The film opens with a montage of images of Manhattan and other parts of New York City accompanied by George Gershwin's Rhapsody in Blue, with Isaac Davis (Woody Allen) narrating drafts of an introduction to a book about a man who loves the city. Isaac is a twice-divorced, 42-year-old television comedy writer dealing with the women in his life who quits his unfulfilling job. He is dating Tracy (Mariel Hemingway), a 17-year-old girl attending the Dalton School. His best friend, college professor Yale Pollack (Michael Murphy), married to Emily (Anne Byrne), is having an affair with Mary Wilkie (Diane Keaton). Mary's ex-husband and former teacher, Jeremiah (Wallace Shawn), also appears. Isaac's ex-wife Jill Davis (Meryl Streep) is writing a confessional book about their marriage. Jill has also since come out of the closet as a lesbian and lives with her partner, Connie (Karen Ludwig).
