A suicidal Hunter "Patch" Adams (Robin Williams) commits himself into a mental institution. Once there, he finds that using humor to help his fellow patients gives him a purpose in life. Because of this he wants to become a medical doctor and two years later enrolls at the Medical College of Virginia (now known as VCU School of Medicine) as the oldest first year student. He questions the school's soulless approach to medical care and clashes with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients as patients and not bond with them as people. Because of this and incidents such as setting up a giant pair of legs during an obstetric conference, he is expelled from the medical school, although he is later reinstated due to his methods actually helping patients improve. Adams encourages medical students to work closely with nurses, learn interviewing skills early, and argues that death should be treated with dignity and sometimes even humor.

The issue before the United States Supreme Court is whether the equal protection clause of the 14th Amendment of the U.S. Constitution mandates the individual states to desegregate public schools; that is, whether the nation's "separate but equal" policy heretofore upheld under the law, is unconstitutional. The issue is placed before the Court by Brown v. Board of Education and its companion case, Briggs v. Elliott. Many of the justices personally believe segregation is morally unacceptable, but have difficulty justifying the idea legally under the 14th Amendment. Marshall and Davis argue their respective cases. Marshall argues the equal protection clause extends far enough to the states to prohibit segregated schools. Davis counters that control of public schools is a "state's rights" issue that Congress never intended to be covered by the 14th Amendment when it was passed.

The story tells the life histories about Saint Peter and Paul of Tarsus after the crucifixion of Jesus, and their individual fates in old Rome in the time of the persecution of Christians. Events in the New Testament Book of Acts by Luke and in the Ecclesiastical History of Eusebius are dramatized and interwoven with the contrasting histories of political intrigues in the public and private lives of the Caesars from Tiberius through Nero related in The Twelve Caesars by Suetonius, together with the fictional drama of the lives of two Jews and two Romans: Caleb the Zealot and his sister Sarah, and Julius Valerius the Imperial Guard and Corinna the patrician woman who has chosen to be a gladiator.
