The story centers on Diana, a voodoo queen using zombies to take revenge on a group of white gangsters. With a former practitioner named Mama Maitresse, she intends on destroying the gangsters who killed her boyfriend and now want her club. Helping her out is the voodoo lord of the dead, Baron Samedi. The zombies in this film more closely resemble the creatures of voodoo legend - i.e., the walking dead who do the bidding of a human master - than the flesh-eating "living dead" ghouls popularized by George A. Romero. According to the film, the zombies are the preserved bodies of slaves brought to the United States from Guinea.

Paul Toombes (Vincent Price) is a successful horror actor whose trademark role was Dr. Death, a skull-faced sadist. During a party in Hollywood showing off his fifth Dr. Death film, he announces his engagement to Ellen Mason (Julie Crosthwait), who gives him an engraved watch as an engagement gift; later that evening, however, adult film producer Oliver Quayle (Robert Quarry) reveals Ellen had worked for him previously. Distraught at Toombes' reaction, Ellen returns to her room, where a masked man in dark garb, similar to Dr. Death's attire, approaches her with a knife. An apologetic Toombes comes in shortly after, only for her decapitated head to fall from her shoulders. The film begins by recapping the events of the previous film, following Dr. Anton Phibes' murderous quest for vengeance against the doctors he blamed for the death of his wife, Victoria. Phibes eluded capture by placing himself in suspended animation in a sarcophagus he shares with the body of his wife, where he would lay in darkness until the moon had entered into proper alignment with the planets, which had not occurred in over two thousand years. Three years later, the conjunction occurs, and Phibes rises from his sarcophagus. Summoning his silent assistant Vulnavia (Valli Kemp, replacing Virginia North), Phibes prepares to take Victoria to Egypt; there, in a hidden tomb, flows the River of Life, promising resurrection for Victoria and eternal life for the two of them. Rising from his basement, Phibes is shocked to discover that his house has been demolished, and a safe containing a scroll of papyrus, showing the way to the River of Life, is now empty.
