A college student discovers her dorm was once a notorious asylum. According to Winthrop's campus newspaper, The Johnsonian, a group of college freshmen with troubled pasts and nightmares are at Richard Miller University for orientation a couple of weeks prior to the beginning of classes. They stumble into an old restricted area and awaken the spirit of a mad doctor, who then proceeds to torture and torment the students in the same way he tortured the patients in 1939 at Burke Asylum. During the film, first String is killed, then Maya, followed by Tommy, school's caretaker Mackey, and Ivy. All are killed by the Doctor. In the end The Doctor chases Madison and Holt through a tunnel, an abandoned factory, and the woods until Madison kills him by stabbing him in the head, causing him to lose all his power and the souls that he has taken. The movie ends with Madison and Holt walking out of the forest finally being able to have closure with their pasts.

High school student Kale Brecht and his father Daniel get into a serious car accident while driving home from a fishing trip, and Daniel dies. One year later, Kale remains inconsolable after his father's death. After Zia commits suicide, he finds himself in an afterlife limbo much the same as life, just slightly worse. Here the color is dim, there are no stars, and no one can smile. Early on, it is revealed that Eugene's car has two idiosyncrasies: a sort of black hole underneath the passenger seat where items that are dropped disappear forever and broken headlights that cannot be fixed by the most adept of mechanics. On their trip, they pick up hitchhiker Mikal, who insists there has been a mistake, and she is seeking the "people in charge" (a.k.a. the PIC) in order to be sent back.

In Japanese society, it is said a curse is created when a person dies in the grip of a powerful rage or sorrow. Those who encounter the evil supernatural force are consumed by it, the curse is born repeatedly and spreads. The original victims of the curse, the Saeki family, haunt their Tokyo suburban house as ghosts and kill anyone who enters. Housewife Kayako Saeki was murdered by her husband Takeo after he discovered she loved another man, their son Toshio and pet cat Mar also being murdered, before Takeo was hung by Kayako’s ghost. In the first film, American social worker Karen Davis tried to burn the house down to stop the curse, but failed, finding herself hospitalised and haunted by Kayako. The film’s chronology is told in a non-linear fashion, taking place in 2004 and 2006 respectfully.
