Older brother Nate fired the deadly shot, but to protect him, younger Skylar, still a minor, takes the blame. Owning most of the town's businesses, the Cavanaugh family itself is wealthy and influential. In retaliation, the family uses their influence to have Skylar set to be tried as an adult, rather than as a minor. Desperate and filled with guilt, Nate breaks Skylar out of the county lock-up and, handcuffed together, the two escape into the local woods. Two deputies are killed in the escape and the Cavanaugh clan, deciding that the sheriff will be unable to act impartially, organize themselves into a search team with orders to shoot on sight. Sheriff Towne joins the search efforts in order to reach his sons before they are shot by others. He is forced to measure the major political debt he owes to the Mayor and the Cavanaugh family, against his oath to uphold the law and the love he has for his two sons.

The plot of the film is largely unchanged from that of Shakespeare's original play. Differences include the modern-day setting, switching Conrade's gender, eliminating several minor roles and consolidating others into Leonato's aide, and expanding Ursula's role by giving her a number of Margaret's scenes. In addition, the film attempts to add background to the relationship between Beatrice and Benedick by showing, in an opening scene, a morning after they apparently slept together. Benedick steals away quietly while Beatrice pretends to be asleep.

In 1954, two U.S. Marshals - Edward "Teddy" Daniels and his new partner, Chuck Aule - travel to the Ashecliffe Hospital for the criminally insane on Shutter Island located in Boston Harbor as part of an investigation into the disappearance of patient Rachel Solando, incarcerated for drowning her three children. The sole clue is a note left by Solando, which reads: "The law of 4; who is 67?" Shortly after arrival, a storm prevents their return to the mainland for several days. Daniels finds the staff confrontational: the lead psychiatrist, Dr. John Cawley, refuses to hand over records of the hospital staff; Solando's doctor, Dr. Lester Sheehan, had left on vacation after her disappearance, and they are barred from searching Ward C and told that the lighthouse on the island has already been searched, so there is no need to search it. When a patient is being interrogated by Daniels, with a subterfuge she sends Aule away for a few seconds and scribbles "RUN" in Daniels' notepad.

By the end of each summer, children from the fundamentalist Catholic community in the suburbs of New Jersey visited Camp Hope. Deep in the woods, away from civilization, the children are taught the ways of cult-like Christianity, the temptations of the flesh, and the horror of Satan. The head priest, who teaches the children these doctrines of fundamentalist Catholicism, has unknowingly brought evil with him by solely focusing on Sin instead of salvation. The priest fills these young minds with the belief that everything they do is an occasion of sin. He delves deep into their psyche asking them things like whether or not they masturbate, because masturbation, according to the Catholic Church, is the same as premarital sex and gives the devil a foothold in their hearts and minds. The priest also goes so far as to call a girl at the camp a whore simply because she was talking to a male member of the camp. These psychological attacks take their toll, and the presence of a demon starts to creep into the minds of the children, especially the main character, Tommy Leary.

Elijah Price (Samuel L. Jackson) is born with Type I osteogenesis imperfecta, a rare disease in which bones break easily. As revealed later in flashbacks, Elijah draws a theory, based on the comic books he has read during his many hospital stays, that if he represents extreme human frailty, perhaps there is someone "unbreakable" at the opposite extreme.
