An eleven-person riot squad races to an insane asylum to quell a recent inmate outbreak and hostage situation. They arrive, only to be caught off guard by a sudden barrage of insane patients. When they've finally fought off a wave, two of their squad members are missing, their communications return only static, and the doors are locked. They're trapped. They break up into three teams to track down the lost members. Strange sounds and religious iconography suggest something beyond insanity has taken over the inmates. Sure enough, a simple rescue mission quickly morphs into relentless life or death battles with possessed. When McGahey, a trained, rational hostage negotiator, discovers the ringleader of this possessed insanity is none other than his brother, it falls on him to stop nothing less than the opening of a portal to Hell.

Before dying, Charles' mother gives him a silver amulet of a howling wolf head, which belonged to his grandfather, who was a Great Hunter of werewolves. Charles escapes death by activating a trap which causes a chandelier to fall and kill the attacking werewolf. Twenty-five years later, Charles is a werewolf bounty hunter, working alongside a band of other hunters to rid the world of all werewolves. Werewolves in this fictional world can inflict lycanthropy. Early in the film we meet Dylan (Shane Curry), approximately 11 years old, sitting on a couch absorbed in a handheld video game and attempting to ignore his father's (Paul Roe) shouts from the kitchen where he is railing at a non-working toaster. We soon learn that rage is his father's natural state; roughly kicked out of the house to "go play", Dylan talks to his next door neighbor, Kylie (Kelly O'Neill), of approximately the same age, about what a "prick" his father is, and the wise decision of his brother to run away two years prior, to which Kylie observes that at least Dylan's father is not in jail like most fathers in the neighborhood, implying that her father is incarcerated. She tells him about the "Sack Man", who she's heard kills kids, but Dylan says that it is just a story, like "Santa and God", used by adults to control kids.
