Late one night, Donald Strachey is meeting a new client in a dark alley—one Paul Hale. Hale is very nervous, and before Strachey can calm what seems like paranoia, an incoming van approaches. Hale hands the private eye a $5,000 check for a retainer just before the speeding van separates the two. The next night, however, Paul Hale turns up dead. The film opens with Abby Russell at a night club where she explains to the audience via a narration that her "job" is to murder cheating men in order to "keep scum off of the streets"- a job that she thoroughly enjoys. She then murders a man by slicing through his femoral artery and then throwing him off the roof of the club. The following day she attends a ceremony where she watches Danni Rodgers, a nurse she had been mentoring, graduate. She then approaches her and meets Danni's mother and stepfather Larry Cook. Despite this joyful moment and all of her preparation, Danni is ill prepared for her first true day of nursing duty and is berated by Dr. Morris for her inability to respond quickly to an emergency situation. Abby views the situation with disgust, as she knows that Dr. Morris is a sadist that enjoys harassing new nurses.

On the outside Zoe Reynard seems to have the perfect life. She has a perfect marriage to her husband Jason and the couple share two kids. Despite this, Zoe is not happy in her marriage. After sex one night with her husband, Zoe is displeased that Jason was done so she wakes up to watch porn and plays with a dildo. A year after the events of Extinction, the Alice clones (Milla Jovovich) attempt to kill Albert Wesker (Shawn Roberts) at the current Umbrella Headquarters, Tokyo. The clones manage to kill the entire branch, but Wesker makes his way to his private charter tiltrotor plane and escapes before activating a bomb, leaving a massive sinkhole. However, the real Alice boarded his aircraft beforehand. As she pulls a gun on him, Wesker injects her with Anti-Virus before the lack of a pilot makes the aircraft crash in the mountains outside Tokyo. Only Alice emerges from the crash alive.
