Years after a zombie apocalypse, survivors have set up outposts across the United States, one of which, in the city of Pittsburgh, Pennsylvania, contains a feudal-like government. Bordered on two sides by rivers and on the third by an electric fence, the city has become a sanctuary, with the rich and powerful living in a luxury high-rise called Fiddler's Green while the rest of the population subsists in squalor. The city's ruler Paul Kaufman (Dennis Hopper) has sponsored Dead Reckoning, a heavily armored vehicle that can travel through the zombie-infested areas with ease. Armed with remote-controlled heavy machine guns and video cameras, Dead Reckoning functions as a moving fireworks launch base, on the premise that zombies are fascinated by fireworks and will stare at them, ignoring their environment. Riley Denbo (Simon Baker), designer and commander of Dead Reckoning, has recently retired. Unlike Kaufman, Riley is respected for his work in protecting the city from danger, as well as for bringing critical food and medical supplies which the citizens can no longer acquire safely themselves.

A year after the events of Extinction, the Alice clones (Milla Jovovich) attempt to kill Albert Wesker (Shawn Roberts) at the current Umbrella Headquarters, Tokyo. The clones manage to kill the entire branch, but Wesker makes his way to his private charter tiltrotor plane and escapes before activating a bomb, leaving a massive sinkhole. However, the real Alice boarded his aircraft beforehand. As she pulls a gun on him, Wesker injects her with Anti-Virus before the lack of a pilot makes the aircraft crash in the mountains outside Tokyo. Only Alice emerges from the crash alive.

In the present-day American Midwest, society struggles to function in the aftermath of a zombie pandemic barely under control. Maggie Vogel (Abigail Breslin) calls her father from a broken city under curfew; her voicemail urges that he not seek her and that she loves him. Her arm was bitten. Knowing she has only weeks before the "necroambulist virus" turns her cannibalistic, she left home to protect her family. Maggie's father Wade (Arnold Schwarzenegger) has searched two weeks, despite her warning. At a government laboratory in rural California, a weaponized version of influenza (called Project Blue) is accidentally released, immediately wiping out everyone on staff except for military policeman Charles Campion and his family, who flee the base. However, Campion is already infected by the superflu, nicknamed "Captain Trips", and spreads it to the outside world. That evening, Campion crashes his car at a gas station in East Texas where Stu Redman (Gary Sinise) and some friends have gathered. When they investigate, they find Campion dying of the flu next to his wife and baby daughter, who are already dead. Campion tells Stu with his dying breath that he was followed from the base by a mysterious figure, and states "You can't outrun the Dark Man". The next day, the U.S. military arrive to quarantine the town. While the other townspeople quickly become ill and die, Stu remains healthy and is confined at a CDC facility in Vermont order to study a possible cure. This proves futile and the superflu rages unchecked, causing civilization to collapse and killing over 99% of the population of the entire world in less than two months.

Three astronauts return to Earth after a nuclear holocaust, although one dies in a crash landing. The two survivors, Newman (Steve Barkett) and Matthews, encounter some mutants before discovering that Los Angeles has been completely destroyed. Seeking shelter, the men take refuge in an abandoned mansion. Newman later encounters a young boy, Chris, hiding in a museum with the curator (Forrest J Ackerman). Barbra (Judith O'Dea) and Johnny Blair (Russell Streiner) drive to rural Pennsylvania for an annual visit to their father's grave. This was their mother's request. Johnny teases, "They're coming to get you, Barbra," noticing Barbra's discomfort. She is then attacked by a strange man (Bill Hinzman). Johnny tries to rescue his sister, but the strange man throws him against a gravestone; Johnny strikes his head on the stone and dies. Barbra flees by car but crashes into a tree. She escapes on foot, with the stranger in pursuit, and later arrives at a farmhouse, where she discovers a woman's mangled corpse. She is confronted by strange menacing figures, running out of the house, like the man in the graveyard. Ben (Duane Jones) takes her into the house. Ben drives the monsters from the house and seals the doors and windows as Barbra slowly descends into shock and insanity.
