Ben Sanderson (Cage) is a Hollywood screenwriter whose alcoholism costs him his job, family, and friends. With nothing left to live for, he moves to Las Vegas to drink himself to death. As he drives drunkenly down the Las Vegas Strip, he nearly hits a woman, Sera (Shue), on the crosswalk. Sera chastises him and walks away. Sera is a prostitute working for an abusive pimp, Yuri Butso (Julian Sands), a Latvian immigrant. Polish mobsters are after Yuri, so he breaks his relationship with Sera in fear that the Poles may hurt her. Yuri is murdered (off-screen) shortly afterwards.

It is May 28, 1976, the last day of school at Lee High School in the suburbs of Austin, Texas. The next year's group of seniors are preparing for the annual hazing of incoming freshmen. Randall "Pink" Floyd, the school's star football player, is asked to sign a pledge promising not to take drugs during the summer or do anything that would "jeopardize the goal of a championship season". When classes end, the incoming freshman boys are hunted down by the seniors and paddled. The incoming freshman girls are also hazed; they are rounded up in the school parking lot by senior girls, covered in mustard, ketchup, flour and raw eggs, and forced to propose to senior boys.

Set in the suburban neighborhood of "Burnfield" in Austin, Texas, five young adults are in the daily habit of hanging out by a garbage dumpster on the corner of a local convenience store, occasionally taunting the foreign clerk, Nazeer Choudhury, who works there. Ann (Sarah Polley) is a hard-working 23-year-old mother with two small daughters, an unemployed husband (Scott Speedman), a mother (Deborah Harry) who sees her life as a failure, and a jailed father whom she has not seen in ten years. Her life changes dramatically when, during a medical checkup following a collapse, she is diagnosed with metastatic ovarian cancer and told she has only two months to live. Deciding not to tell anyone of her condition, using the cover of anemia, Ann makes a list of things to do before she dies. She decides to change her hair, record birthday messages for the girls for every year until they're 18, and tries to set up her husband with another woman. Feeling a longing to experience a life that was never available to her, she seeks out a man to experience how it feels to be in a sexual relationship with someone other than her husband. Her experiment ends up taking an emotional toll when she meets with a man named Lee, who ends up madly in love with her and is left heartbroken when Ann breaks up. He meets with her one last time and says that he will do anything to make her happy, taking care of her daughters and even finding her husband a job. She ends their relationship and never tells him that she is dying. At the end of the film Ann records a message to her husband telling him that she loves him and another one to Lee telling him the same. She then leaves all tapes she has recorded with her doctor asking him to deliver them after her death.

Sydney Wells is a successful classical violinist from Los Angeles who has been blind since she was five years old, caused by an accident with firecrackers. Fifteen years later, after celebrating conductor and pianist Simon McCullogh's birthday during rehearsal, Sydney undergoes a cornea transplant, which causes her eyesight to return, a bit blurry at first. As time goes on, Sydney's vision begins to clear; however, she also begins experiencing terrifying visions, mostly of fire and of people dying. She also sees people that are already dead, on one occasion when a girl passes right through her. Sydney attempts to unravel the mystery of the visions, and also to convince others, primarily her visual therapist and fellow violinist, Paul Faulkner, who helps her in her quest. She knows that she is not going insane.
