Kathryn West, a glamorous American widow, arrives in Italy several weeks after the death of her older, extremely wealthy husband. Danielle et Jean Reynaud ont les apparences d'un couple de grands bourgeois parisiens : il est industriel; riche et accumule les conquêtes car sa femme se refuse à lui. Dans l'appartement au-dessus de chez eux emménage Nicole qui sort d'une relation très difficile avec Klaus. Jean l'attire dans ses bras mais tombe profondément amoureux d'elle. Elle le prévient néanmoins que Klaus cherche à le tuer, à la demande d'un inconnu. Après un échec sur la Riviera, Klaus fait venir Jean dans l'appartement de Nicole et au cours d'une lutte, finit par le poignarder. Il n'a fait qu'exécuter la demande des deux femmes, complices. Le meurtre est maquillé en incendie de la voiture de Jean, dans laquelle la police ne retrouve donc qu'un corps carbonisé. Cependant, Danielle éprouve visions et remords de la disparition de son mari, et finit par être persuadée que Jean n'est pas mort et est complice de Nicole. Alors qu'elle est au téléphone, elle sent sa présence dans l'appartement et meurt d'un coup de feu en prononçant son nom. Sur la déposition de Nicole, la police accepte avec réticence l'hypothèse du suicide, alors que c'est Klaus qui a tiré le coup de feu fatal. Lui et Nicole, qui a hérité de la fortune de Jean, s'envolent pour l'Amérique du Sud. Mais le commissaire est dans l'avion.

The murder of two college students (and one's boyfriend) leads to a manhunt for the killer, whose chief weapon is a red and black ascot to strangle his victims. The film opens with Abby Russell at a night club where she explains to the audience via a narration that her "job" is to murder cheating men in order to "keep scum off of the streets"- a job that she thoroughly enjoys. She then murders a man by slicing through his femoral artery and then throwing him off the roof of the club. The following day she attends a ceremony where she watches Danni Rodgers, a nurse she had been mentoring, graduate. She then approaches her and meets Danni's mother and stepfather Larry Cook. Despite this joyful moment and all of her preparation, Danni is ill prepared for her first true day of nursing duty and is berated by Dr. Morris for her inability to respond quickly to an emergency situation. Abby views the situation with disgust, as she knows that Dr. Morris is a sadist that enjoys harassing new nurses.

Despite the evidence presented by the district attorney (Larry Cedar), Jennifer Garrick (Rosalind Allen), the lawyer defending Vincent Gotto (Lewis Van Bergen), an accused child murderer on death row, believes her client is not guilty, and is hiding the identity of the real killer. A fellow attorney in her office (Ron Canada) explains the presence of a large Pinocchio doll sitting in her chair as belatedly delivered evidence which she had earlier requisitioned (the doll had been buried by her client in his son's grave). Intending to examine it in the hope of finding a clue which might prevent his execution, she brings it home and her emotionally fragile daughter Zoe (Brittany Alyse Smith) mistakes it for a birthday gift. She develops a relationship with the puppet and becomes unbalanced to an even greater degree.

Every year, Stella Fawnskin selects seven members of Sigma Phi Pi, a sorority with chapters all over the United States, to compete for the Fawnskin Grant. On her way to Stella's ranch, which is in the isolated resort town of Grizzly Cove, grant contender Holly Fanning is killed at a deserted rest area. The next day, Detective William Watts is informed by Holly's father, Captain Dan Fanning, that he is being suspended from the LAPD due to his repeated use of excessive force. When Dan, who is protective of his daughter, mentions that Holly has not yet checked in with him, Will offers to go to Grizzly Cove to keep an eye on her, in exchange for Dan helping him get back on the force.

A young woman walks through the hallways of an apartment building, looking for "Mr. Gunther". She walks into a room in the attic and the door closes and locks behind her. Inside the room, she finds rats in cages, as well as a caged woman (Sally Brown). Karl Gunther (Klaus Kinski) appears and says of the caged woman: "She can't talk. I cut her tongue off." Karl presses a button and a blade is shoved through the woman's back and sticks out of her chest, killing her.
