The film is notable in resembling a low-budget Hong Kong action film, despite its American production and cast. A review in Variety noted that characters from a Chinese myth are given Japanese names and played by caucasians.
In a 2006 interview, the writer/producer Glen Hartford claims he based the story on "a piece of mythological history, from over 4000 years ago", and calls the movie an "Asian film".

Neil Shaw (Snipes) is an operative for the United Nations's covert dirty-tricks squad, using espionage and quasi-ethical tactics to secure peace and cooperation. Truck driver Jack Burton (Kurt Russell) and his friend, restaurant owner Wang Chi (Dennis Dun), go to the airport to pick up Wang's fiancée Miao Yin (Suzee Pai), who is arriving from China. A Chinese street gang, the Lords of Death, tries to kidnap another Chinese girl at the airport who is being met by her friend Gracie Law (Kim Cattrall), intending to sell her as a sex slave. After Jack intervenes, they take Miao Yin instead. In Jack's big-rig truck, he and Wang track the Lords of Death to the back alleys of Chinatown, where they find a funeral procession that quickly erupts into a street fight between the Chang Sing and Wing Kong, two ancient Chinese societies. When "The Three Storms" (Thunder, Rain, and Lightning) appear, slaughtering the Chang Sing, Jack tries to escape but runs over Lo Pan (James Hong), a powerful and legendary sorcerer, as well as the leader of the Wing Kong. Horrified, Jack exits his truck, only to find Lo Pan, who is merely annoyed. Wang hurriedly guides Jack through the alleys, escaping the carnage and mayhem, but Jack's truck is stolen.

The story centers around Max Havoc (Mickey Hardt), an ex-kickboxing champion known as "Mad Max", turned globetrotting sports photographer. Max quit kickboxing after accidentally killing a fellow boxer during an unlikely comeback in the ring, but still suffers from flashbacks to the fight. After a bar scuffle over a biker girl (Nikki Ziering), his agent (Diego Walraff) sends him to Guam for a publicity photo shoot. There, Max encounters Tahsi (Richard Roundtree), his former kickboxing coach, now an antiques dealer, and promises to catch up with him later. While photographing an outrigger canoe race from a jet ski, Max rescues Christy Goody (Tawney Sablan), a vacationer who was about to be unwittingly run over by the canoes. In the process, he knocks over one of the canoes, earning the wrath its head rower, Moko (Pyun veteran Vincent Klyn). He is also admonished by Jane (Joanna Krupa), Christy's sister, for his brazen driving. However, she later apologizes and agrees to a dinner date.

Larry Chin, a Chinese martial arts expert, sneaks into the United States at a San Francisco port, looking for his brother Wei Chin who had disappeared several years prior, shortly after Larry's wife died in an accident. Bloodsport III reintroduces the character Alex Cardo (Daniel Bernhardt) from Bloodsport II. As Alex gambles in a casino, masked men steal money and a package from the casino, but not before Alex beats up several of the men. After the robbery, the casino owner convinces Alex to retrieve the package (a bag of diamonds) from the robbers, since they belong to a mob boss named Duvalier (John Rhys-Davies). Alex does so, and Duvalier invites him to a dinner party he's hosting as thanks.
