An onscreen text warns of the superstitious belief in a vorvolaka, a malevolent force in human form. The film proper begins during the Balkan Wars of 1912. While his troops are burying their dead, General Pherides (Karloff) and American reporter Oliver Davis (Marc Cramer) visit the Isle of the Dead to pay their respects to the General's long-dead wife. They discover the crypt despoiled; hearing a woman singing on the supposedly uninhabited island, they set out to find her. They also find retired Swiss archeologist Dr. Aubrecht (Jason Robards, Sr.), his Greek housekeeper Madame Kyra (Helen Thimig), British diplomat Mr. St. Aubyn (Alan Napier) and his pale and sickly wife (Katherine Emery), her youthful Greek companion Thea (Ellen Drew), and an English tinsmith., 1h19

Set in 1761 in London, the film focuses on events at St. Mary's of Bethlehem Asylum, a fictionalized version of Bethlem Royal Hospital, also known as "Bedlam." After an acquaintance of aristocrat Lord Mortimer dies in an attempt to escape from the asylum, apothecary general Master George Sims (played by Karloff, a fictionalized version of an infamous head physician at Bethlem, John Monro) appeases Mortimer by having his "loonies" put on a show for him. Mortified by the treatment of the patients, Mortimer's protégé Nell Bowen (Lee) seeks the help of Whig politician John Wilkes to reform the asylum. Mortimer and Sims conspire to commit Nell to the asylum, where her initial fears of the fellow inmates do not sway her sympathetic commitment to improving their conditions. Frustrated by Nell's progress with the inmates, Sims threatens her with his strongest "cure" but his attempt is thwarted by the very inmates that Nell helped. Ultimately, Sims is literally "deposed" and Nell is rescued by her Quaker friend who had counselled her through the whole process., 1h9

Tom Merriam (Russell Wade), a young merchant marine officer, joins the crew of the ship Altair. At first, all seems well and Merriam bonds with the captain, Will Stone (Richard Dix). The ship, already shorthanded due to the death of a crew member before it left port, loses another ("the Greek") when he develops appendicitis. (Taking direction over the ship's radio, the captain is to perform the appendectomy, but he is unable to make the incision. Instead, Merriam successfully removes the sailor's appendix, but – feeling he should be loyal to the captain and spare him embarrassment – swears the radio operator to secrecy. Afterward, the captain has a self-serving explanation for his failure., 1h40

Mr. Morgan (Cooper) washes up on an island in the South Pacific, ruled by Pastor Corbett (Barry Jones), a missionary who rules as a religious despot. Morgan stays on the island despite his conflicting with Corbett and has an illegitimate child, Turia (Moira Walker), by island native Maeva (Haynes). After her death, he leaves the island, but also leaves her daughter behind. He returns to the island shortly before World War II, and his daughter falls in love with an American pilot who crash-lands on the island. An interesting irony is that when Morgan first arrived, Corbett tried to force him to leave the island before getting one of the girls pregnant. Then the same thing happens again when his daughter, Turia, tries to have an affair with the American pilot. Morgan intervenes, and makes the pilot leave. In the last scene his daughter forgives him.
