The film begins in 2010, with a 65-year-old Howard Marks going onto a stage in front of a packed theatre to great applause. Marks asks if there are any plain-clothes policemen sat in the audience, to which the reply is negative. Then, while lighting up a joint, Marks asks "who here is a dope (marijuana) smoker?", to even greater applause. The film then diverts into Marks' internal monologue as he recounts his life.

Johan "JW" Westlund is a man living in Stockholm and is a promising student at the Stockholm School of Economics. JW is originally from the Norrland region in Sweden where his father, who has a slight drinking problem, works in a saw mill, while his mother works in a jobs agency. JW is racked by the disappearance of his sister, Camilla, four years earlier. Titta Di Girolamo is a middle-aged loner who has spent the last eight years living in an upmarket hotel in Lugano, Switzerland. Seventeen-year-old Colombian girl María Álvarez works in sweat shop-like conditions at a flower plantation. Immediately after the events of the previous film, Chev Chelios (Jason Statham) lands in the middle of an intersection after falling out of a helicopter. August 2005, New Orleans, Louisiana. Terence McDonagh (Nicolas Cage) is a New Orleans Police Sergeant. While cleaning out a locker after Hurricane Katrina, he notices that a prisoner may not have been transferred. When he finds the prisoner about to drown, he mocks him before eventually jumping in the water to save him.
