Predatory lenders Simone and Eddie awaken on either side of a room split by a cage containing a scale, wearing head harnesses with screws poised to pierce their temples. They learn via videotape that the one who sets more flesh weight on their tray will survive. Eddie cuts fat from his torso, but Simone chops off her arm, which tilts the scale in her favor; the timer expires, and Eddie is killed by the screws. While he reviews the game's footage, Detective Lt. Mark Hoffman is called to the scene by FBI Agent Dan Erickson, who found Agent Peter Strahm's fingerprints around the room. Also present is Agent Lindsey Perez, whose survival was covered up by Erickson for her protection. They learn from Dr. Adam Heffner, who performed all of the autopsies for the Jigsaw case, that the knife used to cut the jigsaw piece from Eddie's flesh was only used on Seth Baxter before then. He discovers a hidden exit with a microcassette recorder, which urges him to stay in the sickroom. He ignores it and is attacked in the tunnel by a pig-masked figure, waking up with his head sealed in a box which quickly fills with water; he survives by performing a tracheotomy using his pen. The film begins with the sentencing of Jean-Baptiste Grenouille (Ben Whishaw), a notorious murderer. Between the reading of the sentence and the execution, the story of his life is told in flashback, beginning with his abandonment at birth in a French fish market. Raised in an orphanage, Grenouille grows into a strangely detached boy with a superhuman sense of smell. After growing to maturity as a tanner's apprentice, he makes his first delivery to Paris, where he revels in the new odors. He focuses on a redheaded girl (Karoline Herfurth) selling yellow plums, following her and repeatedly attempting to sniff her, but startles her with his behavior. To prevent her from crying out, he covers the girl's mouth and unintentionally suffocates her. After realizing that she is dead, he strips her body naked and smells her all over, becoming distraught when her scent fades. Afterwards, Grenouille is haunted by the desire to recreate the girl's aroma.

Their teenaged son, Jesse, has a nightmare about being stranded on a school bus with two girls and being stalked by a violent killer. After getting into a fight with a boy named Grady during gym class, Coach Schneider makes them stay after class and they eventually become friends. Lisa comes to visit Jesse after school and they discover a diary from Nancy Thompson detailing her nightmares which are strikingly similar to Jesse's.
